3、omplaints6Student Grievance Procedure6IntroductionThe Well Animal Institute LLP, (hereinafter referred to as the School) offers the finest technology and equipment for student training and is staffed with qualified, approved instructors.The School is owned by Dr. Anna (Turie) Norman and Cindy Lloyd.

12、ayEntrance RequirementsThe school does not discriminate based on race, sex, religion, ethnic origin, or disability.It is up to each individual to check the laws in their State, Providence or Country regarding Anesthesia-Free teeth cleaningProspective students must have a high school diploma or equiv

13、alency diploma to be accepted for enrollment. If applicants are beyond the age of compulsory school attendance in Colorado and do not possess a diploma or equivalency diploma, they may complete an ability to benefit test approved by the Colorado Department of Higher Education. The school does not ad

14、minister the test, but will provide information on availability when requested. It should be understood that this is a physical job. The applicant should be in good health and physical condition and able to sit for long periods of time on the floor. You will have to complete up to 20 dogs to pass th

15、e class. EnrollmentProspective students may enroll up to two week prior to the class date. Late enrollments may not be accepted. A minimum of 1 student must attend to conduct the course.Postponement of Start DatePostponement of a starting date, whether at the request of the school or the student, re

16、quires a written agreement signed by the student and the school. The agreement must set forth:a. Whether the postponement is for the convenience of the school or the student, and;b. A deadline for the new start date, beyond which the start date will not be postponed.If the course is not commenced, o

17、r the student fails to attend by the new start date set forth in the agreement, the student will be entitled to an appropriate refund of prepaid tuition and fees within 30 days of the deadline of the new start date set forth in the agreement, determined in accordance with the schools refund policy a

18、nd all applicable laws and rules concerning the Private Occupational Education Act of 1981.Placement AssistanceThe Well Animal Institute does not offer employment assistance to graduates. We will provide information on marketing ideas for your new business. We make no guarantee, expressed or implied

19、, of future employment. Current law prohibits any school from guaranteeing job placement as an inducement to enroll students. You will be required to check the laws in your state on whether you need to do this under veterinary supervision.Attendance RequirementsStudents are expected to arrive on tim

20、e for class with proper materials. An overall attendance rate of at least 95% is required. Instructors may request your withdrawal from a course or program if absences or tardiness exceed 20%.Students who are unable to continue classes for medical reasons or severe personal problems will be required

21、 to withdraw in writing from class but will be allowed to continue the days they missed on the next class offering.Progress PolicyStudents must maintain an 85% grade point average. The student may be terminated if the teachers feel he/she is not making satisfactory progress during the course. Termin

22、ation shall be at the school Directors discretion. The Director has final authority and shall notify the student of the final decision.Grading System96 100 = AExcellent86 95= BAbove Average76 85= CAverage66 75= DBelow AverageUnder 66 = UUnsatisfactoryThe hands on portion of the course is critical to

23、 passing the course and receiving your certificate of completion. Students will receive daily feedback during the course as to their progress.Conduct PolicyAll students are expected to act maturely and are required to respect other students and faculty members. Possession of weapons, illegal drugs,

24、and alcohol of any kind are not allowed at any time on school property. Any violation of school policies may result in permanent dismissal from school.DismissalAny student may be dismissed for violations of rules and regulations of the school, as set forth in school publications. A student also may

25、be withdrawn from classes if he or she does not prepare sufficiently, neglects assignments, or makes unsatisfactory progress. The director, after consultation with all parties involved, makes the final decision.The Director of Education may temporarily suspend students whose conduct is disruptive or

26、 unacceptable to the academic setting. After appropriate counseling, students who demonstrate a genuine desire to learn and conform to school standards of conduct, may be allowed to resume attendance. The director will review each case and decide upon re-admittance.FacilitiesThe school classes are h

27、eld at: A Dog Rescue for the first day of class, then various locations- you will attend our clinics scheduled for the week. Class locations can be found on the Well Animal Institute website. 29、s CreditsThe school does not except credit from previous training or experience.The school does not guarantee transfer ability of its credits to another institution unless there is a written agreement with another institution.Student Grievance ProcedureAny complaint should be in writing to the schoo

30、ls Director.Student ComplaintsStudent Complaints should be brought to the attention of the School Director to attempt resolution. The Director and student are to follow the grievance procedures according to school policy printed in the school catalog. If satisfactory resolution cannot be reached bet

31、ween the student and the school, the student may file a written complaint online with the Colorado Division of Private Occupational Schools at: /dpos or by requesting a complaint form at (303) 866-2723. All student complaints submitted to the Division must be in writing and

32、shall be filed within two years after the student discontinues training at the school.Complaints of deceptive trade or sales practices(1) A person claiming pecuniary loss as a result of a deceptive trade or sales practice, pursuant to section 12-49-117, by a school or agent shall first exhaust all c

33、omplaint and appeals processes available at the school. If the persons complaint is not resolved to the persons satisfaction, the person may file with the board a written complaint against the school or agent.Refund PolicyStudents not accepted to the school are entitled to all moneys paid. Students

34、who cancel this contract by notifying the school in writing within three (3) business days are entitled to a full refund of all tuition and fees paid. Students, who withdraw after three (3) business days, but before commencement of classes, are entitled to a full refund of all tuition and fees paid

35、except the maximum cancellation charge of $150.00 or 25% of the contract price, whichever is less. In the case of students withdrawing with a written notice after commencement of classes, the school will retain a cancellation charge ($150.00) plus a percentage of tuition and fees, which is as descri

36、bed in the table below. The refund is based on the last date of recorded attendance. (Each day is counted as one entire day of class; Partial days up to noon are days.
