2、請表 記名卡個人記名卡、單位主附卡的發(fā)卡，由電子銀行部定期做記名卡批量制卡，下載打卡文件，交制卡廠制卡。 3、卡片入庫 成品卡制成后，由財務方案部要求制卡企業(yè)負責將實物送到各旗縣聯(lián)社，并以系統(tǒng)記賬和登記成品卡出入庫登記簿。 4、卡片領用 旗縣聯(lián)社領用成品卡時，持加蓋業(yè)務公章的預制卡申請清單到自治區(qū)財務部方案領用成品卡。領用時，自治區(qū)聯(lián)社方案財務部專管人員將時間、數(shù)量、卡號等事項及時記錄成品卡出入庫登記簿，各旗縣聯(lián)社接收到制卡企業(yè)分送的成品卡實物時要逐一進展核對，如發(fā)現(xiàn)有重號、錯號、漏號等情況，及時上報財務方案部，在確定無誤前方可簽收，領用的成品卡應予妥善保管，做好登記入庫。財務方案部接到電子銀
