6、救援體系是否建立并實(shí)施演練。 Supervise the establishment of emergency response system and emergency drills. 4. 定義 DEFINITION 4.1特種設(shè)備：是指涉及生命安全、危險(xiǎn)性較大的鍋爐、壓力容器（含氣瓶，下同）、壓力管道、電梯、起重機(jī)械、客運(yùn)索道、大型游樂(lè)設(shè)施。 High risk equipment: those that have impact on safety, high risk boiler, pressurized vessel (gas cylinder), pressurized pipe

8、or, hoisting machines, transportation cableway, and large recreational facilities and vehicles within the plant etc.4.3壓力容器：Pressurized vessel 是指盛裝氣體或者液體，承載一定壓力的密閉設(shè)備，其范圍規(guī)定為最高工作壓力大于或者等于（表壓），且壓力與容積的乘積大于或者等于·的氣體、液化氣體和最高工作溫度高于或者等于標(biāo)準(zhǔn)沸點(diǎn)的液體的固定式容器和移動(dòng)式容器；盛裝公稱(chēng)工作壓力大于或者等于（表壓），且壓力與容積的乘積大于或者等于·的氣體、液化氣體和

9、標(biāo)準(zhǔn)沸點(diǎn)等于或者低于液體的氣瓶；氧艙等。It refers to air tight devices that store gas or liquid, with maximum pressure higher or equal to 0.1MPa (specified pressure), and stationary and mobile type of vessels that have pressure and volume higher or equal to 2.5MP.L and can contain liquid gas with highest working temper

Gas cylinder and oxygen chamber etc that can contain gas with nominal working pressure higher or equal to 0.2MPa (surface pressure) with boiling point higher to or lower than 60oC, and have pressure and volume larger or equal to 1.0 MPa.4.4壓力管道 P

13、t.4.5電梯 Elevator 是指動(dòng)力驅(qū)動(dòng)，利用沿剛性導(dǎo)軌運(yùn)行的箱體或者沿固定線路運(yùn)行的梯級(jí)（踏步），進(jìn)行升降或者平行運(yùn)送人、貨物的機(jī)電設(shè)備，包括載人（貨）電梯、自動(dòng)扶梯、自動(dòng)人行道等。It refers to enclosure that is raised or lowered in a vertical shaft by mechanical force or steps that operates in a fix line; which is used to transport people or freight vertically or horizontally. 16、safety protection devices.5. 程序 PROCEDURES 5.1特種設(shè)備崗位的建立 Establishment of Positions using High Risk Equipment 特種設(shè)備的購(gòu)入應(yīng)保持以下文件完整有效：設(shè)計(jì)文件、制造單位、產(chǎn)品質(zhì)量、合格證明、使用維護(hù)說(shuō)明等文件以及安裝技術(shù)文件的資料。Purchase of high risk equipment should come with the following documents: design drawings, manufacturer information, product qualit

17、y, validity certification, usage and maintenance instruction manual and installation procedures.5.1.2特種設(shè)備在投入使用前或者投入使用后日內(nèi)，設(shè)施部門(mén)應(yīng)當(dāng)向直轄市或者設(shè)區(qū)的市的特種設(shè)備安全監(jiān)督管理部門(mén)登記。登記標(biāo)志應(yīng)當(dāng)置于或者附著于該特種設(shè)備的顯著位置。Facilities department should register with the high risk equipment safety inspection & management department in the dis

18、trict or estate before or 30 days after using the high risk equipment. Registered label should be placed or attached on one visible part of the high risk equipment.5.1.3設(shè)施組應(yīng)建立特種設(shè)備安全技術(shù)檔案。安全技術(shù)檔案應(yīng)當(dāng)包括以下內(nèi)容：Facilities department should establish safety working files for high risk equipment. The file shoul

19、d contain the following contents: 1) 特種設(shè)備的設(shè)計(jì)文件、制造單位、產(chǎn)品質(zhì)量合格證明、使用維護(hù)說(shuō)明等文件以及安裝技術(shù)文件和資料;Design drawings, manufacturer information, product quality, validity certification, usage and maintenance instruction manual and installation procedures;2) 特種設(shè)備的定期檢驗(yàn)和定期自行檢查的記錄； Periodic inspection and self-check record

20、s of the high risk equipment;3) 特種設(shè)備的日常使用狀況記錄；Records of daily usage of the high risk equipment;4) 特種設(shè)備及其安全附件、安全保護(hù)裝置、測(cè)量調(diào)控裝置及有關(guān)附屬儀器儀表的日常維護(hù)保養(yǎng)記錄；Daily maintenance records of the high risk equipment and its safety accessories, safety protective device, measuring and adjustment device and any attached in

21、strument and meter;5)特種設(shè)備運(yùn)行故障和事故記錄。Records of malfunctions and accidents caused by the equipment. 5.1.4嚴(yán)格執(zhí)行操作規(guī)程，操作人員不得擅自拆除和損壞特種設(shè)備安全附件。 The operators must follow the operation guidelines strictly and not allowed to dismantle and damage the safety accessories that are attached to the high risk equipme

22、nt. 5.2特種作業(yè) High Risk Operations 特種作業(yè)人員持證作業(yè)。The employees involved in high risk operations must put on the identification passes while working.1各部門(mén)新增特種作業(yè)崗位或人員應(yīng)通知人事部門(mén)，人事部門(mén)建立特種設(shè)備作業(yè)人員管理檔案。Individual department must inform the human resource department on any new high risk operation or new employee that

23、 handles high risk operation. The human resources department will establish personal files for such employees.2人事部門(mén)對(duì)作業(yè)人員進(jìn)行持續(xù)安全教育和培訓(xùn)。 Human resources department will carry out continuous safety education and training for operators. 特種設(shè)備作業(yè)現(xiàn)場(chǎng)應(yīng)制訂特種設(shè)備操作規(guī)程和有關(guān)安全管理制度，包括“叉車(chē)安全操作管理規(guī)定”等。 High risk operation sit

24、e should have the customized operating procedures for high risk equipment and relevant safety management procedures, including “Regulations on Forklift Operations”.5.2.3特種設(shè)備操作人員嚴(yán)格執(zhí)行操作規(guī)程，不得擅自拆除和損壞特種設(shè)備安全附件。The high risk equipment operators must follow the operation guidelines strictly and not allowed

25、to dismantle and damage the safety accessories that are attached to the high risk equipment.5.2.4操作人員在正常操作狀態(tài)下發(fā)現(xiàn)異常情況，應(yīng)立即匯報(bào)主管，不得擅自處理。Operators should immediately report to supervisor and not solve by themselves if any abnormal situation is detected. 5.2.5保持特種設(shè)備整潔和周?chē)h(huán)境的清潔衛(wèi)生。 Maintain cleanliness of hig

26、h risk equipment and hygiene of surrounding environment.新增特種設(shè)備或作業(yè)崗位應(yīng)對(duì)其可能導(dǎo)致的重大環(huán)境影響、重大危險(xiǎn)源和危害因素實(shí)施評(píng)價(jià)，評(píng)價(jià)依照EHS一般作業(yè)與設(shè)施設(shè)備管理程序?qū)嵤?Evaluation should be done on newly added high risk equipment or operation for presence of any probable significant environment aspect and hazardous source. Evaluation is carried o

27、ut according to Procedures for Managing Equipment & Operations5.3特種設(shè)備的檢查檢驗(yàn) Examination and Inspection of High Risk Equipment 設(shè)施部門(mén)對(duì)在用特種設(shè)備應(yīng)當(dāng)至少每月進(jìn)行一次自行檢查，并作出記錄。在對(duì)在用特種設(shè)備進(jìn)行自行檢查和日常維護(hù)保養(yǎng)時(shí)發(fā)現(xiàn)異常情況的，應(yīng)當(dāng)及時(shí)處理。 Facilities department should carry out monthly checks on existing high risk equipment, and maintain r

28、ecords of such activities. Any abnormality found during the monthly checks or maintenance must be dealt with immediately.5.3.1.1設(shè)施部門(mén)應(yīng)當(dāng)對(duì)在用特種設(shè)備的安全附件、安全保護(hù)裝置、測(cè)量調(diào)控裝置及有關(guān)附屬儀器儀表進(jìn)行定期校驗(yàn)、檢修，并作出記錄。Facilities department should carry out regular checks and maintenance on safety accessories, safety protective devi

29、ce, measuring and adjustment device and any attached instrument and meter of existing high risk equipment; and maintain records of such activities.5.3.2設(shè)施部門(mén)應(yīng)當(dāng)按照安全技術(shù)規(guī)范的定期檢驗(yàn)要求，在安全檢驗(yàn)合格有效期屆滿(mǎn)前個(gè)月向特種設(shè)備檢驗(yàn)檢測(cè)機(jī)構(gòu)提出定期檢驗(yàn)要求。未經(jīng)定期檢驗(yàn)或者檢驗(yàn)不合格的特種設(shè)備，不得繼續(xù)使用Facilities department should request for periodic checks by the hi

30、gh risk equipment inspection authorities according to safety work regulations one month before the expiration of the safety certification. Operation on the high risk equipment that has not undergone the periodic check or fails the periodic check must be stopped.5.3.3特種設(shè)備出現(xiàn)故障或者發(fā)生異常情況，使用部門(mén)應(yīng)當(dāng)對(duì)其進(jìn)行全面檢查，消

31、除事故隱患后，方可重新投入使用。 The department using the high risk equipment must do an investigation when the equipment is malfunction or when there is an abnormal situation. Operation can only be resumed after elimination of all risk factors.5.3.4電梯的日常維護(hù)保養(yǎng)必須由取得資質(zhì)許可的安裝、改造、維修單位或者電梯制造單位進(jìn)行。Daily maintenance of eleva

32、tor should be carried out by technicians with valid certificate or manufacturer of the elevator.5.3.5各部門(mén)監(jiān)督特種作業(yè)人員持證上崗和按章操作。Individual department must ensure that the operators of high risk equipment put on identification passes and comply with the operating procedures while working.5.3.6沒(méi)有依照工廠特種作業(yè)管理制

33、度實(shí)施的依照“員工手冊(cè)”實(shí)施處理。Those employees who do not comply with the high risk operating procedures will be dealt with according to the “Employees Handbook”5.4特種設(shè)備的報(bào)廢 Procedure of Disposing of High Risk Equipment 特種設(shè)備存在嚴(yán)重事故隱患，無(wú)改造、維修價(jià)值，或者超過(guò)安全技術(shù)規(guī)范規(guī)定使用年限，使用部門(mén)將信息提交設(shè)施部門(mén)申請(qǐng)報(bào)廢When the high risk equipment is beyond m

34、aintenance and repair, there is a risk of causing serious accident or the equipment has exceeded the usage life as specified by safety work regulations, the responsible department will seek the approval of the facilities department to dispose the equipment.報(bào)廢設(shè)備由設(shè)施部門(mén)向原登記的特種設(shè)備安全監(jiān)督管理部門(mén)辦理注銷(xiāo)。The facilities department will write to the High Risk Equipment Safety Inspection & Management Department that had registered the equipment to cancel the registration.5.5特種作業(yè)應(yīng)急預(yù)案ERP for High Risk Operations 特種設(shè)備的事故應(yīng)急措施和救援預(yù)案依照對(duì)應(yīng)特種設(shè)備安全管理規(guī)定實(shí)施。The ERP to respond to accident caused by high r
