16、情況;績(jī)效總體目標(biāo)和階段性績(jī)效目標(biāo)的完成情況;項(xiàng)目總投入,財(cái)政撥款、自籌資金落實(shí)、實(shí)際支出情況;主要經(jīng)濟(jì)、社會(huì)、環(huán)境效益及可持續(xù)影響等;財(cái)務(wù)管理情況;項(xiàng)目管理制度及執(zhí)行情況等。(二)項(xiàng)目績(jī)效及評(píng)價(jià)結(jié)論對(duì)項(xiàng)目實(shí)施后所取得的實(shí)際績(jī)效進(jìn)行認(rèn)真梳理與總結(jié),并根據(jù)評(píng)價(jià)得分給出相應(yīng)評(píng)價(jià)等次。(三)評(píng)價(jià)發(fā)現(xiàn)的問(wèn)題對(duì)項(xiàng)目實(shí)施和資金使用過(guò)程中存在的有關(guān)問(wèn)題進(jìn)行總結(jié)與歸納。(四)相關(guān)意見(jiàn)與建議對(duì)評(píng)價(jià)過(guò)程中發(fā)現(xiàn)的問(wèn)題進(jìn)行認(rèn)真分析與說(shuō)明,并針對(duì)有關(guān)問(wèn)題提出相應(yīng)的整改意見(jiàn)與建議。(五)附件評(píng)價(jià)機(jī)構(gòu)認(rèn)為需要作為評(píng)價(jià)報(bào)告附件的有關(guān)文件、資料等,以進(jìn)一步解釋和證明報(bào)告所反映的相關(guān)內(nèi)容。五、其他要求評(píng)價(jià)報(bào)告統(tǒng)一使用A4紙打印、
