11、):The progress report is the tool used to communicate to Cal OES on the status of the project during the specified timeframes. Please provide a detailed narrative covering, at a minimum, the following areas for the appropriate reporting period. The information provided on progress reports may be sha
12、red with other agencies. Please use as many pages as necessary. Do not provide any confidential information as this is a public document. FIRST/SECOND QUARTER PROGRESS REPORT Discuss any delays and/or problems the project has experienced in meeting the terms of the Grant Award Agreement during the a
13、pplicable reporting period. For each component, discuss those activities supporting each objective which are not currently operational. For each component, describe the projects activities and accomplishments to date (if applicable, attach press releases and news articles). For each component, discu
14、ss those objectives which have not been implemented, why, and what steps will be taken to complete implementation. Discuss the source documentation designed to track the projects statistical information (i.e., each components tracking format, statistical summary forms, etc.). Discuss, in detail, any
15、 significant accomplishments. Please be specific, as this information may be used to compile reports to the legislature. THIRD/FOURTH QUARTER PROGRESS REPORT Discuss the impact the project has had on the human trafficking problem identified in the projects problem statement according to the Grant Aw
16、ard Agreement. For each component, summarize the activities and accomplishments for the entire grant period (if applicable, attach press releases and news articles). Discuss, in detail, any significant accomplishments. Please be specific, as this information may be used to compile reports to the leg
17、islature. Describe the progress the project has had in meeting the expected objectives.STATISTICAL REPORTING FORMSAttached are statistical reporting forms for each component of the project. Please use these forms to compile statistical data for each quarter. The data must reflect the activities of a
18、ll personnel (grant funded and non-grant funded) assigned to the project. All data must be supported by source documentation which is to be retained by the grant recipient and made available to Cal OES upon request. Compile all quarterly figures in the “total to date” column. Attach additional pages
19、 to further clarify the data.PROJECT NARRATIVE (please attach additional pages as needed): Instruction Sheet for CompletingHF12 Progress ReportProgress Reports ask for statistical information from each team working on Anti-Human Trafficking Task Force (HF) Program cases so it is important that each
20、of these teams have a common definition for terms used in the reports. It is also important for teams to receive additional clarification related to the reporting requirements contained in the progress reports. The following information should promote a shared understanding among team members so tha
21、t all teams report similar data.UDEFINITIONSU:UDisruptedU:Impeding the normal and effective operation of the targeted organization.UDismantledU: Destruction of the organizations leadership, financial base, and supply network such that the organization is incapable of operating and/or reconstituting
22、itself.UOperationsU:This includes, but is not limited to: Setting up Craigs List “out call” dates, where the john/offender responds to an undercover task force member at a location such as a vehicle, hotel, or apartment; utilizing an undercover task force member on the street posing as a prostitute;
23、 or, conducting a raid on a massage parlor.“UCommunityU” and “UCountry of OriginU” relate to a domestic vs. international offender. UComprehensive ServicesU:An array of services that can be offered to a human trafficking victim that will address needs identified at the time of intake and service pla
24、nning (a list of services is located in the progress report, and is UNOTU limited to those items).Cal OES recognizes that there is a disparity with regard to the focus of the existing nine Anti-Human Trafficking Task Forces in the state of California. In an effort to best coordinate the efforts of t
