1范圍

本文件規(guī)定了礦山生態(tài)保護(hù)修復(fù)工程質(zhì)量驗(yàn)收的總體要求、工程類別劃分及工程質(zhì)量驗(yàn)收標(biāo)準(zhǔn)、單

項(xiàng)工程質(zhì)量驗(yàn)收程序、土石方及邊坡防護(hù)、排水與灌溉、引水工程、濕地修復(fù)、礦區(qū)土地復(fù)墾、礦業(yè)遺

跡保護(hù)、生態(tài)修復(fù)監(jiān)測(cè)、自然修復(fù)及輔助再生等單位工程的驗(yàn)收要求。

本文件適用于歷史遺留礦山、廢棄礦山等礦山生態(tài)保護(hù)修復(fù)工程的質(zhì)量管理和驗(yàn)收。生產(chǎn)礦山可參

照?qǐng)?zhí)行。

3術(shù)語(yǔ)和定義

下列術(shù)語(yǔ)和定義適用于本文件。

3.1

檢驗(yàn)

對(duì)被檢驗(yàn)項(xiàng)目的特征和性能進(jìn)行量測(cè)、檢查、試驗(yàn)等,并將結(jié)果與標(biāo)準(zhǔn)規(guī)定要求進(jìn)行比較,以確定

每項(xiàng)性能是否合格所進(jìn)行的活動(dòng)。

3.2

隱蔽工程

施工完畢后將被其后續(xù)工程所遮蓋或掩埋而無法或難以對(duì)它進(jìn)行檢查的工序或分項(xiàng)工程。

3.3

單項(xiàng)工程

具有獨(dú)立的設(shè)計(jì)文件,能夠獨(dú)立組織施工,竣工后可獨(dú)立發(fā)揮整體效益的工程。

3.4

單位工程

具備獨(dú)立施工條件并能形成獨(dú)立使用功能的工程。

3.5

分部工程

單位工程的組成部分,按照專業(yè)性質(zhì)、工程部位、使用功能劃分的工程。

3.6

分項(xiàng)工程

分部工程的組成部分,按照不同的施工方法、材料、工序及施工范圍劃分的工程。

3.7

礦山生態(tài)修復(fù)

依靠自然力量或者工程措施,對(duì)礦產(chǎn)資源開發(fā)造成的礦山生態(tài)問題進(jìn)行修復(fù)、恢復(fù)或改善礦山生態(tài)

系統(tǒng),建立新的動(dòng)態(tài)平衡的過程。

3.8

礦山地質(zhì)災(zāi)害

礦業(yè)活動(dòng)引發(fā)或加劇危害人民生命和財(cái)產(chǎn)安全的崩塌、滑坡、泥(廢)石流、地面塌陷、地裂縫、

地面沉陷等。

3.9

地形地貌景觀破壞

礦業(yè)活動(dòng)改變了原有的地形條件與地貌特征,造成山體破損、巖石裸露、植被破壞等自然景觀破壞

的現(xiàn)象。

3.10

水資源水生態(tài)破壞

礦業(yè)活動(dòng)造成地下水資源枯竭、地表水漏失,或由于礦業(yè)活動(dòng)造成地表水、地下水生態(tài)服務(wù)功能減

退等現(xiàn)象。

3.11

土地資源占損

因礦業(yè)活動(dòng)挖損、塌陷、壓占等造成的土地資源破壞或毀損。

3.12

礦山土地復(fù)墾

采取工程措施對(duì)因采礦活動(dòng)而占損的土地進(jìn)行綜合整治,使其恢復(fù)到可供利用狀態(tài)的活動(dòng)。

3.13

礦業(yè)遺跡

礦業(yè)開發(fā)過程中遺留下來的蹤跡和與采礦活動(dòng)相關(guān)的實(shí)物。

4總體要求

4.1工程質(zhì)量驗(yàn)收等級(jí)分為“合格”和“不合格”,驗(yàn)收“不合格”的工程,經(jīng)整改后重新驗(yàn)收;

4.2各階段完工工程必須滿足設(shè)計(jì)要求,上一階段驗(yàn)收資料是下一階段驗(yàn)收的依據(jù);

4.3工程整體驗(yàn)收后,應(yīng)將各階段驗(yàn)收資料進(jìn)行整理、分類標(biāo)注,統(tǒng)一編號(hào)歸檔;

4.4礦山地質(zhì)災(zāi)害防治工程各階段質(zhì)量驗(yàn)收應(yīng)符合DB43/T1358的有關(guān)規(guī)定;

4.5礦山生態(tài)保護(hù)修復(fù)工程質(zhì)量驗(yàn)收,除應(yīng)符合本文件外,尚應(yīng)符合國(guó)家現(xiàn)行有關(guān)標(biāo)準(zhǔn)的規(guī)定。

5工程類別劃分及工程質(zhì)量驗(yàn)收標(biāo)準(zhǔn)

5.1工程類別劃分

5.1.1根據(jù)生態(tài)保護(hù)修復(fù)工程建設(shè)任務(wù)、現(xiàn)場(chǎng)施工管理和質(zhì)量驗(yàn)收需要,應(yīng)在施工準(zhǔn)備階段進(jìn)行生態(tài)

保護(hù)修復(fù)工程類別劃分。

5.1.2生態(tài)保護(hù)修復(fù)工程類別劃分為自然修復(fù)、輔助再生、生態(tài)重建工程三大類。

5.1.3生態(tài)重建工程類別按工程級(jí)別劃分為單項(xiàng)工程、單位工程、分部工程和分項(xiàng)工程。工程類別劃

分表見附錄A。

5.2工程質(zhì)量驗(yàn)收標(biāo)準(zhǔn)

5.2.1合格標(biāo)準(zhǔn)

5.2.1.1分項(xiàng)工程質(zhì)量驗(yàn)收合格應(yīng)符合以下要求:

a)分項(xiàng)工程所用的原材料、半成品、成品及附件應(yīng)符合相應(yīng)規(guī)范質(zhì)量標(biāo)準(zhǔn)的要求;

b)分項(xiàng)工程施工中質(zhì)量檢查、見證取樣檢測(cè)和隱蔽工程部分質(zhì)量驗(yàn)收記錄應(yīng)完整、真實(shí);

c)分項(xiàng)工程質(zhì)量驗(yàn)收應(yīng)綜合工序的現(xiàn)場(chǎng)質(zhì)量檢查記錄合格情況;

d)實(shí)測(cè)項(xiàng)目驗(yàn)收抽查檢驗(yàn)應(yīng)全部合格;

e)外觀質(zhì)量應(yīng)符合基本要求。

5.2.1.2分部工程質(zhì)量驗(yàn)收合格應(yīng)符合以下要求:

a)分部工程所含分項(xiàng)工程的質(zhì)量驗(yàn)收均應(yīng)合格;

b)質(zhì)量控制資料應(yīng)完整;

c)有關(guān)安全及主要功能的抽樣檢驗(yàn)結(jié)果應(yīng)符合相應(yīng)規(guī)定;

d)外觀質(zhì)量應(yīng)符合基本要求。

5.2.1.3單位工程質(zhì)量驗(yàn)收合格應(yīng)符合以下要求:

a)單位(子單位)工程所含分部(子分部)工程的質(zhì)量驗(yàn)收應(yīng)全部合格;

b)質(zhì)量控制資料應(yīng)完整;

c)所含分部工程中有關(guān)安全、生態(tài)環(huán)境保護(hù)和主要使用功能檢驗(yàn)資料應(yīng)完整;

d)主要使用功能的抽查結(jié)果應(yīng)符合本文件或相關(guān)驗(yàn)收規(guī)范的規(guī)定;

e)外觀質(zhì)量應(yīng)符合基本要求。

5.2.1.4單項(xiàng)工程質(zhì)量驗(yàn)收合格應(yīng)符合以下要求:

a)項(xiàng)目所含單位工程的質(zhì)量均應(yīng)合格;

b)質(zhì)量控制資料應(yīng)完整;

c)所含單位工程外觀質(zhì)量應(yīng)符合基本要求;

d)監(jiān)測(cè)成果及數(shù)據(jù)分析應(yīng)符合國(guó)家和行業(yè)技術(shù)標(biāo)準(zhǔn)要求。

5.3工程質(zhì)量驗(yàn)收記錄

5.3.1單位工程主要質(zhì)量控制資料見附錄B。

5.3.2施工現(xiàn)場(chǎng)質(zhì)量檢查記錄應(yīng)按本文件附錄C的規(guī)定填寫。

5.3.3分項(xiàng)工程、分部工程、單位工程質(zhì)量驗(yàn)收記錄分別按本文件附錄D、附錄E、附錄F的規(guī)定填寫。

5.3.4單位工程、分部工程外觀質(zhì)量檢查記錄應(yīng)按本文件附錄G的規(guī)定填寫。

5.3.5單項(xiàng)工程質(zhì)量驗(yàn)收記錄應(yīng)按本文件附錄H的規(guī)定填寫。

5.3.8參建各方(施工單位、監(jiān)理單位、設(shè)計(jì)單位、實(shí)施單位)的單項(xiàng)工程質(zhì)量驗(yàn)收資料見附錄I。

5.3.6單項(xiàng)工程基本情況應(yīng)按本文件附錄J的規(guī)定填寫。

5.3.7單項(xiàng)工程質(zhì)量驗(yàn)收意見應(yīng)按本文件附錄K的規(guī)定填寫。

5.4工程質(zhì)量“不合格”的處理

5.4.1分項(xiàng)、分部、單位工程質(zhì)量驗(yàn)收不滿足5.2.1.1~5.2.1.3節(jié)要求的驗(yàn)收為不合格。

5.4.2經(jīng)返修或返工的分項(xiàng)工程,應(yīng)重新進(jìn)行驗(yàn)收。

5.4.3經(jīng)返修或返工仍不能滿足安全或使用要求的分項(xiàng)工程、分部工程、單位工程,不準(zhǔn)許驗(yàn)收。

5.4.4施工單位應(yīng)在工程質(zhì)量缺陷期內(nèi),按國(guó)家相關(guān)法律法規(guī)要求和承包合同約定的要求,對(duì)工程質(zhì)

量缺陷負(fù)責(zé)整改。

6工程質(zhì)量驗(yàn)收程序

6.1隱蔽工程

6.1.1施工單位應(yīng)在每道工序完成后及時(shí)進(jìn)行自檢,并做好隱蔽工程現(xiàn)場(chǎng)記錄(包括文字記錄、索描

圖、可視化影像等),自檢不合格的,不能進(jìn)入下道工序施工。

6.1.2隱蔽工程在隱蔽前應(yīng)由監(jiān)理單位進(jìn)行驗(yàn)收,并形成驗(yàn)收文件。外觀質(zhì)量應(yīng)由現(xiàn)場(chǎng)代表及相關(guān)驗(yàn)

收人員通過現(xiàn)場(chǎng)檢查共同確認(rèn)。

6.2分項(xiàng)工程

6.2.1分項(xiàng)工程按設(shè)計(jì)文件要求完工后,施工單位應(yīng)在自檢合格的基礎(chǔ)上,向監(jiān)理機(jī)構(gòu)提交分項(xiàng)工程

質(zhì)量報(bào)驗(yàn)申請(qǐng)表和相關(guān)資料。

6.2.2監(jiān)理機(jī)構(gòu)收到分項(xiàng)工程質(zhì)量報(bào)驗(yàn)申請(qǐng)表和相關(guān)資料后,應(yīng)由監(jiān)理工程師組織實(shí)施單位現(xiàn)場(chǎng)代表、

施工單位項(xiàng)目技術(shù)負(fù)責(zé)人等進(jìn)行驗(yàn)收;涉及安全的重要隱蔽工程和分項(xiàng)工程,實(shí)施、勘查、設(shè)計(jì)、施工

單位項(xiàng)目負(fù)責(zé)人和總監(jiān)理工程師共同參加驗(yàn)收,并確認(rèn)質(zhì)量等級(jí)。

6.2.3參加驗(yàn)收人員均應(yīng)在質(zhì)量驗(yàn)收意見欄內(nèi)簽字,相關(guān)驗(yàn)收單位應(yīng)蓋章。

6.3分部工程

6.3.1分部工程按設(shè)計(jì)文件要求完工后,施工單位應(yīng)在自檢合格的基礎(chǔ)上,向項(xiàng)目監(jiān)理機(jī)構(gòu)提交分部

工程報(bào)驗(yàn)申請(qǐng)和相關(guān)資料。

6.3.2項(xiàng)目監(jiān)理機(jī)構(gòu)收到分部工程驗(yàn)收申請(qǐng)和相關(guān)資料后,應(yīng)由監(jiān)理工程師會(huì)同實(shí)施單位項(xiàng)目現(xiàn)場(chǎng)負(fù)

責(zé)人組織施工單位項(xiàng)目負(fù)責(zé)人、技術(shù)負(fù)責(zé)人、質(zhì)量負(fù)責(zé)人等進(jìn)行驗(yàn)收,并確認(rèn)質(zhì)量等級(jí)。

6.3.3參加驗(yàn)收人員均應(yīng)在質(zhì)量驗(yàn)收意見欄內(nèi)簽字,相關(guān)驗(yàn)收單位應(yīng)蓋章。

6.4單位工程

6.4.1單位工程按設(shè)計(jì)文件要求完工后,施工單位應(yīng)在自檢合格的基礎(chǔ)上,向項(xiàng)目監(jiān)理機(jī)構(gòu)提交單位

工程報(bào)驗(yàn)申請(qǐng)表和相關(guān)資料。

6.4.2項(xiàng)目監(jiān)理機(jī)構(gòu)收到單位工程報(bào)驗(yàn)申請(qǐng)表和相關(guān)資料后,應(yīng)由總監(jiān)理工程師組織實(shí)施單位現(xiàn)場(chǎng)項(xiàng)

目負(fù)責(zé)人及勘查、設(shè)計(jì)、施工等單位負(fù)責(zé)人進(jìn)行驗(yàn)收,并確認(rèn)質(zhì)量等級(jí)。

6.4.3參加驗(yàn)收人員均應(yīng)在質(zhì)量驗(yàn)收意見欄內(nèi)簽字,相關(guān)驗(yàn)收單位應(yīng)蓋章。

6.5單項(xiàng)工程

6.5.1項(xiàng)目工程按設(shè)計(jì)文件和合同要求完工后,施工單位應(yīng)在自檢合格基礎(chǔ)上,向?qū)嵤﹩挝惶峤豁?xiàng)目

工程驗(yàn)收申請(qǐng)報(bào)告。

6.5.2實(shí)施單位收到施工單位項(xiàng)目工程驗(yàn)收申請(qǐng)報(bào)告后,由實(shí)施單位組織施工單位項(xiàng)目負(fù)責(zé)人、技術(shù)

負(fù)責(zé)人以及勘查、設(shè)計(jì)單位項(xiàng)目負(fù)責(zé)人、項(xiàng)目總監(jiān)理工程師及相關(guān)專業(yè)專家進(jìn)行項(xiàng)目工程驗(yàn)收。

6.5.3項(xiàng)目工程驗(yàn)收應(yīng)對(duì)項(xiàng)目建設(shè)任務(wù)完成情況、工程質(zhì)量、技術(shù)檔案、質(zhì)量問題整改等方面進(jìn)行驗(yàn)

收。

6.5.4項(xiàng)目工程驗(yàn)收應(yīng)形成工程驗(yàn)收意見。

6.5.5參加驗(yàn)收人員均應(yīng)在質(zhì)量驗(yàn)收意見欄內(nèi)簽字,相關(guān)驗(yàn)收單位應(yīng)蓋章。

7土石方及邊坡防護(hù)

7.1土石方

7.1.1邊坡修整

7.1.1.1邊坡修整應(yīng)符合以下基本要求:

a)邊坡整體穩(wěn)定,無因整坡破壞而降低原有邊坡穩(wěn)定性;

b)設(shè)置施工臨時(shí)排水系統(tǒng),避免邊坡沖刷、梯級(jí)平臺(tái)積水等;

c)按照設(shè)計(jì)要求完成截排水設(shè)施,無嚴(yán)重水土流失;

d)在設(shè)定取土區(qū)內(nèi)合理取土,無亂采濫挖,完工后應(yīng)按要求對(duì)取土場(chǎng)和棄土場(chǎng)進(jìn)行修復(fù)。

7.1.1.2邊坡修整實(shí)測(cè)項(xiàng)目見表1。
