1、有機(jī)產(chǎn)品認(rèn)證文件資料清單Documentation List for Organic Product Certification (食用菌栽培和加工)(Edible Mushroom Cultivation & Processing)申請單位應(yīng)提供以下文件資料并按序號編排、裝訂后提交:The applicant shall submit the following documents bound in the order as below:食用菌栽培Edible Mushroom Cultivation1. 有機(jī)產(chǎn)品認(rèn)證申請書Application for Organic Produc
2、t Certification2. 有機(jī)產(chǎn)品認(rèn)證調(diào)查表(食用菌栽培)Organic Product Certification Questionnaire (Edible Mushroom Cultivation)3. 營業(yè)執(zhí)照副本復(fù)印件和組織機(jī)構(gòu)代碼證復(fù)印件Copy of business license (duplicate) and copy of organization code certificate4. 土地使用權(quán)證明文件Documentary evidence of land use right要求:Requirements:(1)土地承租或流轉(zhuǎn)的,提供土地承租/流轉(zhuǎn)合同(其中
3、應(yīng)體現(xiàn)范圍、面積、詳細(xì)地址、合同有效期限);Contract of land lease/ transfer (in which scope, area, detailed address and contract life shall be specified) shall be provided, if the land is acquired through lease or transfer;(2)公司/合作社+農(nóng)戶組織模式的,應(yīng)提供申請單位與生產(chǎn)者簽訂的有機(jī)種植合同(合同中應(yīng)體現(xiàn)范圍、面積、詳細(xì)地址、合同有效期限及有機(jī)生產(chǎn)的相關(guān)要求);Organic farming contract
4、 (in which scope, area, detailed address, contract life and requirements of organic production shall be specified) signed by the applicant and the producer shall be provided, if the business is conducted through the organization pattern of company/ cooperative+ farmers;(3)新開墾的土地必須出具縣級以上政府部門開具的開發(fā)批復(fù)。D
5、evelopment reply issued by the government organ above the county level shall be presented, if the land is newly reclaimed.5. 農(nóng)戶清單及農(nóng)戶管理制度(適用于公司/合作社+農(nóng)戶的組織模式)List of farmers and farmer management system (applicable to the organization pattern of company/ cooperative+ farmers)要求:Requirements:(1)農(nóng)戶清單應(yīng)至少包
6、含農(nóng)戶姓名、身份證號、地塊編號、種植品種、面積等;Farmers name, ID Card No., land number, plant species and area must be specified in the list of farmers;(2)農(nóng)戶管理制度應(yīng)體現(xiàn)有機(jī)生產(chǎn)的相關(guān)要求和對農(nóng)戶有效的管理。Requirements of organic production and effective management of farmers shall be embodied in the farmer management system.6. 產(chǎn)地(基地)區(qū)域位置圖Locat
7、ion plan for the place of origin (base)要求:Requirements:(1)基地所在地的行政圖(市、縣或鄉(xiāng)的行政圖,表明基地所在的位置);Administrative map of the location of the base (the administrative map of the city, county or town that shows the location of the base);(2)地塊分布圖(多地塊和分布分散情況下,應(yīng)提供全部地塊分布情況的地圖);Parcel distribution diagram (the map s
8、howing the distribution of all land parcels shall be provided in case of several parcels or disperse distribution);(3)地塊圖(需體現(xiàn)出每個(gè)地塊的形狀、面積、周邊土地利用情況、緩沖帶設(shè)置情況、主要標(biāo)示物、河流、水井或其他水源的位置等信息)。Parcel diagram (which shall show the shape, area, surrounding land utilization, setup of buffer strips, major markers, loc
9、ations of rivers, wells or other water sources of every parcel).7. 本年度食用菌灌溉用水質(zhì)量監(jiān)(檢)測報(bào)告Quality monitoring (testing) report for irrigation water of edible mushrooom for the current year要求:Requirements:(1)環(huán)境監(jiān)(檢)測機(jī)構(gòu)應(yīng)具有相關(guān)資質(zhì);The environmental monitoring (testing) organization shall be of relevant qualific
10、ations;(2)依據(jù)GB5749生活飲用水標(biāo)準(zhǔn)要求檢測。The testing shall be conducted in compliance with GB5749 domestic drinking water standard. (3)有機(jī)產(chǎn)品生產(chǎn)、加工規(guī)劃Organic product production and processing plan(4)有機(jī)生產(chǎn)質(zhì)量管理手冊O(shè)rganic production quality management manual要求:Requirement:請參考GB/T19630.4有機(jī)產(chǎn)品 第4部分:管理體系4.2.4編制Please refer
11、 to 4.2.4 Formulation, Part 4: Management System, Organic Products (GB/T 19630.4)10. 有機(jī)生產(chǎn)、經(jīng)營操作規(guī)程Operating specifications on organic production and operation要求:Requirements:(1)食用菌栽培技術(shù)規(guī)程;Technical specifications on edible mushroom cultivation; 包括:a.菌種的來源及處理、接種方法;b.栽培基質(zhì)的成分、來源及制作規(guī)程;c. 食用菌栽培區(qū)域的消毒、滅菌、病蟲害防
12、治規(guī)程;Including: a. Source of strain and methods of treatment and planting; b. Ingredients, source and making regulations on culture substrate; c. Regulations on sterilization, disinfection and pest control of edible mushroom cultivation area;(2)防止有機(jī)生產(chǎn)、加工和經(jīng)營過程中受禁用物質(zhì)污染所采取的預(yù)防措施;Measures for preventing p
13、ollution of banned substances in organic production, processing and operation;(3)食用菌產(chǎn)品收獲規(guī)程及收獲后運(yùn)輸、儲(chǔ)藏等各道工序的規(guī)程;Regulations on harvest and after-harvest transportation and temporary storage of edible mushroom products;(4)運(yùn)輸工具、機(jī)械設(shè)備及倉儲(chǔ)設(shè)施的維護(hù)、清潔規(guī)程;Regulaitons on maintaining and cleaning transportation facil
14、ities, mechanical equipment and warehousing facilities;(5)防止有機(jī)產(chǎn)品與非有機(jī)產(chǎn)品混雜所采取的措施(存在平行生產(chǎn)的企業(yè)須提交);Measures for preventing the mixing of organic products and non-organic products (necessarily required for any enterprise with parallel production);(6)標(biāo)簽及生產(chǎn)批號的管理規(guī)程;Regulations on managing labels and batch num
15、bers;(7)員工福利與勞動(dòng)保護(hù)規(guī)程。Reguations on employee benefits and labor protection.(8)所購買菌種的購買單據(jù)、非轉(zhuǎn)基因證明、常規(guī)菌種未經(jīng)禁用物質(zhì)處理的證明Receipts for purchase of strain and proof for no genetical modification and no banned-substance treatment of conventional strain12. 所購買的各種生產(chǎn)資料(消毒劑、生物農(nóng)藥等)的購買單據(jù)、產(chǎn)品說明書Purchase bills for producti

17、le mushroom15. 食用菌收獲記錄Edible mushroom harvest records 16. 銷售記錄及有機(jī)標(biāo)識的使用管理記錄Sales record and organic label use and management records 17. 培訓(xùn)記錄(時(shí)間、培訓(xùn)內(nèi)容、參與人員、授課人員)Training records (time, content, participants and teachers)18. 內(nèi)部檢查記錄Internal inspection records19. 產(chǎn)品召回記錄Product recall records20. 客戶投訴處理記錄R
18、ecords of treating customer complaints21. 介紹和說明基地情況的相關(guān)照片Pictures introducing and describing the base有機(jī)產(chǎn)品加工Organic Product Processing對于收獲后需進(jìn)行加工的產(chǎn)品(以QS證為準(zhǔn)),還需同時(shí)提交以下資料。The documents as mentioned below shall be provided additionally, where the harvested products are to be processed (subject to QS certif

QS證書復(fù)印件Copy of QS certificate3. 加工廠行政位置圖Administrative location plan of the processing plant4. 加工廠區(qū)平面圖及車間設(shè)備位置圖(應(yīng)標(biāo)明加工廠周邊環(huán)境、加工、包裝車間、倉庫及相關(guān)設(shè)備的分布)Floor plan of the processing plant and location plan of
20、workshop equipment (Surrounding environment, processing and packaging workshops, warehouses and allocation of relevant equipment shall be shown)5. 本年度加工用水的檢測報(bào)告(適用時(shí))Processing water testing report for the current year (if applicable)6. 如為委托加工,提供委托加工合同Processing consignment contract shall be provided,
21、 if the processing is entrusted.7. 有機(jī)產(chǎn)品加工規(guī)程Organic product processing regulations(1)各產(chǎn)品加工工藝流程圖及各環(huán)節(jié)操作規(guī)程;Process flow diagram for every kind of products and operating regulations for every step;(2)產(chǎn)品的包裝材料、方法和儲(chǔ)藏等環(huán)節(jié)規(guī)程;Regualtions on packaging materials and methods and storage of products;(3)廢水、廢渣等廢棄物的處理
22、規(guī)程;Regulations on disposing such wastes as waste water and waste residue;(4)加工廠衛(wèi)生管理與有害生物控制規(guī)程。Regulations on sanitary management and pest control for the processing plant.8. 原料的運(yùn)輸記錄(運(yùn)輸工具、時(shí)間、原料名稱、批次號、數(shù)量)Records of raw material transportation (transportation facilities, time, name of raw material, batc
23、h number and quantity)9. 產(chǎn)品加工記錄(工序名稱、時(shí)間、原料名稱、批次號、加工數(shù)量、出成率、成品數(shù)量)Product processing records (process name, time, names of raw materials, batch number, quantity of processing, yield and quantity of finished products)10.添加劑、加工助劑的購買單據(jù)、產(chǎn)品說明書及臺(tái)帳記錄(來源、購買數(shù)量、使用數(shù)量、庫存數(shù)量等)Purchase receipts for additives and proc
24、essing aids, product manuals, and standing book records (source, quantity of purchase, quantity of use and quantity in stock)11.產(chǎn)品出入庫記錄Records of products into and out of the warehouse12.機(jī)械設(shè)備清潔記錄(時(shí)間、設(shè)備名稱、清掃清潔方法、藥劑名稱、用量)Records of cleaning mechanical equipment (time, name of equipment, cleaning or sweeping method, name of agent and dosage)13.加工
