3、PREI on-line; 2、遵守賽寶認證中心(CEPREI)的認證規(guī)章和程序；支付認證所需的申請試驗，工廠審查及其它有關(guān)的費用；妥當保管型式試驗報告、工廠檢查報告和認證變更確認等認證的相關(guān)資料，以備監(jiān)督檢查使用；賽寶認證中心(CEPREI)將不擔(dān)當獲得產(chǎn)品合格認證的生產(chǎn)者（制造商）或銷售商應(yīng)擔(dān)當?shù)娜魏畏韶?zé)任；/We declare that we will follow the rules and procedures of the CEPREI and make payment for the fees arising from the application, testing, in

4、spection and other services, keep well type test report, factory inspection report, materials approving for change of certified product and relevant documents of certification in order to conduct the factory follow up inspection.CEPREI shall not bear any corresponding legal liabilities which should

5、be assumed by Manufacturer and Seller with product certificate; 3、所生產(chǎn)的本申請所涉及的產(chǎn)品符合申請認證產(chǎn)品的國家標準及其它相關(guān)標準或規(guī)定，我公司對此次聲明完全負責(zé)；/We declare with full responsibility that the relevant product(s) produced by our factory are in conformity with the GB standards and other standards or regulations; 4、始終遵守認證方案的支配的有關(guān)規(guī)定

6、；/we undertake to always comply with the relevant provisions of the certification program; 5、為進行評價作出必要的支配，包括審查文件、進入全部的區(qū)域、查閱全部的記錄（包括內(nèi)部審核報告）和評價所需人員（例如檢驗、檢查、評定、監(jiān)督、認證打算）和解決投訴的有關(guān)規(guī)定；/we undertake to make all necessary arrangements for the conduct of the evaluation, including provision for examining docume

9、PREI may consider misleading or unauthorized; 8、當證書被暫停或撤銷時，應(yīng)馬上停止涉及認證內(nèi)容的廣告，并按賽寶認證中心(CEPREI)要求交回全部認證文件；/we undertake to upon suspension orcancellation of certification, discontinue the use of all advertising matter that contains any reference thereto and return any certification documents as required

10、by CEPREI; 9、認證僅用于表明獲準認證的產(chǎn)品符合特定標準；/we undertake to use certification only to indicate that products are certified as being in conformity with specified standards; 10、確保不采納誤導(dǎo)的方式使用或部分使用認證證書和報告；/we undertaketo endeavor to ensure that no certificate or report nor any part thereof is used in a misleading

11、 manner; 11、在傳播媒體中對 CCC 認證內(nèi)容的引用，應(yīng)符合賽寶認證中心(CEPREI)的要求；/we undertake to in making reference to our CCC certification in communication media such as documents, brochures or advertising, comply with the requirements of CEPREI; 12、此申請中的產(chǎn)品符合國家其它相關(guān)法律法規(guī)的要求后方可出廠銷售。/The product in this application can be sold

13、*-* 2、請用中、英兩種文字填寫認證托付人、生產(chǎn)者（制造商）、生產(chǎn)企業(yè)和認證產(chǎn)品的名稱。/Please fill in the names of the applicant, factories and products in both Chinese and English. 3、有關(guān)賽寶認證中心(CEPREI)CCC 認證的公開文件可通過上網(wǎng)獵取，網(wǎng)址是：l/Published documents on CEPREICCC certification can be acquired from CEPREI website: l 4、假如申請 CCC 認證，同時也申請 CB 測試證書，請?zhí)?/p>

14、寫完"認證托付人承諾'后，連續(xù)填寫 CB 測試申請書。/If CB application applied at the same time withCCC application, please complete Declaration of the Applicant and then to fill in CB application form. 5、CCC 認證與 CB 認證同時申請時，生產(chǎn)者（制造商）必需相同。/If CCC certification and CB certification apply at the same time, the manufact

15、urer of themmustbe the same. 6、CB 申請書以英文填寫為準，相同的內(nèi)容可以復(fù)制。/The English content of CB application is admitted, the same contents of it can make duplicate. 7、認證托付人獲證后有關(guān)證書注銷、暫停（恢復(fù)）、撤銷等重要信息，賽寶認證中心將以"通知書'形式按上述"認證托付人'信息中的內(nèi)容以"掛號信'方式進行郵寄、告之。如持證人證書內(nèi)容（認證托付人/生產(chǎn)者（制造商）/生產(chǎn)企業(yè)名稱、地址等）發(fā)生變化，請準時

16、申請證書變更。除證書內(nèi)容外，認證托付人/代理機構(gòu)/生產(chǎn)者（制造商）/生產(chǎn)企業(yè)其他內(nèi)容發(fā)生變化，請準時聯(lián)系受理工程師修改。/CEPREI will take in the form of "Notice' to notify certificate holder the information of cancellation, suspension (resumption), and withdrawal. The "Notice' will send by registered letter according to theaddress of applic

If there is any change on certificate contents (applicant/manufacturer/name or address of factory), please apply the certificate modification in time, except that, if the change on applicant /agency/manufacturer /factory, please contact with engineer in time. 8、認證托付人獲證后有關(guān)證書注銷、暫停（恢復(fù)）

18、、撤銷的信息，請登錄賽寶認證中心網(wǎng)站（http:/*）或認監(jiān)委網(wǎng)站(http:/*）進行查詢。同時認證托付人需將證書注銷、暫停（恢復(fù)）、撤銷的信息準時通知與之相關(guān)的生產(chǎn)者（制造商）、生產(chǎn)企業(yè)，并根據(jù)強制性產(chǎn)品認證的相關(guān)規(guī)定執(zhí)行。/The certificate holder can land on CEPREI website (http:/*) or Certification and Accreditation Administration of the Peoples Republic of China website (http:/*) to get the information on certificates cancellation, suspension (resumption) and withdrawal. In addition, the applicant should notice the related manufacturer or factory the information of certificates cancellation, suspension (resumption) and withdrawal, and comply with related CCC requirements. 9、獲證后，請仔細
