1總則

1.0.1首次質(zhì)量監(jiān)督檢查應(yīng)在升壓站基礎(chǔ)混凝土澆筑前、光伏組件支架基礎(chǔ)混凝土澆筑前(無基礎(chǔ)時

為支架安裝前)進行;在既有建(構(gòu))筑物上增設(shè)光伏發(fā)電系統(tǒng)的工程,應(yīng)在既有建(構(gòu))筑物結(jié)構(gòu)

的安全性確認合格后進行。

2監(jiān)督檢查前應(yīng)具備的條件

2.0.1進場的責任主體單位項目組織機構(gòu)已建立,人員已到位。

2.0.2已進場的工程原材料質(zhì)量證明文件齊全,按規(guī)定復(fù)檢合格。

2.0.3施工組織設(shè)計已審批。

2.0.4施工現(xiàn)場供水、供電、通訊、道路等滿足施工需要。

3責任主體質(zhì)量行為的監(jiān)督檢查

3.1建設(shè)單位

3.1.1工程項目按規(guī)定基本完成招投標并與承包商簽訂合同。

3.1.2項目負責人已經(jīng)單位法定代表人授權(quán),并已簽署工程質(zhì)量終身責任承諾書。

3.1.3質(zhì)量管理組織機構(gòu)已建立,質(zhì)量管理人員已到位。

3.1.4質(zhì)量管理制度已發(fā)布。

3.1.5監(jiān)理規(guī)劃、施工組織設(shè)計已審批。

3.1.6工程采用的專業(yè)標準清單已審批。

3.1.7工程建設(shè)有關(guān)質(zhì)量強制性標準(強制性條文)實施管理措施已制定。

3.1.8組織完成設(shè)計交底及施工圖會檢。

3.1.9工程項目開工手續(xù)已辦理。

3.1.10施工質(zhì)量驗收范圍劃分表已審批。

3.1.11項目主要備案內(nèi)容發(fā)生變更的,已告知項目備案機關(guān)并完成信息修改。

3.2勘察單位

3.2.1項目負責人已經(jīng)單位法定代表人授權(quán),并已簽署工程質(zhì)量終身責任承諾書;專業(yè)人員具有相應(yīng)資格。

3.2.2勘察報告已正式出具。

3.2.3勘察代表工作到位,處理問題及時。

3.2.4按規(guī)定參加工程質(zhì)量驗收。

3.2.5工程建設(shè)有關(guān)質(zhì)量強制性標準(強制性條文)實施計劃已建立,相關(guān)管理制度已執(zhí)行。

3.2.6勘察技術(shù)交底已完成,交底記錄齊全。

3.3設(shè)計單位

3.3.1項目負責人已經(jīng)單位法定代表人授權(quán),并已簽署工程質(zhì)量終身責任承諾書。專業(yè)人員具有相應(yīng)

資格。

3.3.2工程設(shè)計更改控制程序、現(xiàn)場服務(wù)管理制度已建立。

3.3.3設(shè)計圖紙交付進度能保證連續(xù)施工。

3.3.4設(shè)計交底已完成,交底記錄齊全。

3.3.5設(shè)計代表到位,處理問題及時。

3.3.6按規(guī)定參加工程質(zhì)量驗收。

3.3.7工程建設(shè)有關(guān)質(zhì)量強制性標準(強制性條文)實施計劃已建立,相關(guān)管理制度已執(zhí)行。

3.4監(jiān)理單位

3.4.1總監(jiān)理工程師已經(jīng)單位法定代表人授權(quán),并已簽署工程質(zhì)量終身責任承諾書。

3.4.2項目監(jiān)理機構(gòu)健全,監(jiān)理人員專業(yè)配備及數(shù)量符合合同約定,資格滿足要求;人員變更手續(xù)符

合相關(guān)規(guī)定。

3.4.3監(jiān)理規(guī)劃、監(jiān)理實施細則已編制完成,審批手續(xù)齊全。

3.4.4檢測設(shè)備、計量工器具配置滿足需要,經(jīng)檢定(校準)合格且在有效期內(nèi)。

3.4.5按規(guī)定對施工現(xiàn)場質(zhì)量管理進行檢查。

3.4.6組織或參加材料進場和設(shè)備、構(gòu)配件開箱檢查驗收以及原材料見證取樣。

3.4.7按專業(yè)組織完善并審核施工質(zhì)量驗收范圍劃分表,報建設(shè)單位批準后監(jiān)督實施。

3.4.8本階段應(yīng)執(zhí)行的工程建設(shè)有關(guān)質(zhì)量強制性標準(強制性條文)實施情況已檢查。

3.4.9按規(guī)定完成報審(報驗)文件的審核(審查或驗收)。

3.5施工單位

3.5.1項目經(jīng)理已經(jīng)單位法定代表人授權(quán),并已簽署工程質(zhì)量終身責任承諾書。

3.5.2項目部組織機構(gòu)健全,項目經(jīng)理資格滿足要求,專業(yè)人員配置滿足工程實際需要;項目經(jīng)理變

更手續(xù)符合相關(guān)規(guī)定。

3.5.3質(zhì)量管理制度已發(fā)布。

3.5.4特種作業(yè)人員持證上崗,質(zhì)量檢查員經(jīng)培訓(xùn)合格。

3.5.5施工組織設(shè)計、施工方案已報審。技術(shù)交底記錄齊全。

3.5.6檢測設(shè)備、計量工器具配置滿足需要,經(jīng)檢定(校準)合格且在有效期內(nèi)。

3.5.7檢測試驗計劃已制定并報審。

3.5.8單位工程開工申請已經(jīng)審批。

3.5.9專業(yè)綠色施工措施已制定并報審。

3.5.10工程建設(shè)有關(guān)質(zhì)量強制性標準(強制性條文)實施計劃已建立,相關(guān)管理制度已執(zhí)行。

3.5.11供應(yīng)方有關(guān)資料已按規(guī)定報審。進場工程材料、成品、半成品、構(gòu)配件的質(zhì)量證明文件齊全。

3.5.12施工質(zhì)量驗收范圍已劃分,并審批完成。

3.6檢驗檢測機構(gòu)

3.6.1檢驗檢測機構(gòu)(含現(xiàn)場試驗室)已經(jīng)監(jiān)理、建設(shè)單位審核,審核內(nèi)容包括檢驗檢測機構(gòu)是否取

得市場監(jiān)督管理部門等相關(guān)部門資質(zhì)認定證書,檢驗檢測能力范圍是否涵蓋被檢測項目類別等。

3.6.2檢測人員資格符合規(guī)定。

3.6.3檢測設(shè)備、計量工器具配置滿足需要,經(jīng)檢定(校準)合格且在有效期內(nèi)。

3.6.4檢測依據(jù)正確、有效,檢測報告及時、規(guī)范。

4施工現(xiàn)場條件和工程實體質(zhì)量監(jiān)督檢查

4.0.1測量定位基準點驗收合格,廠區(qū)平面控制網(wǎng)、高程控制網(wǎng)、主要建(構(gòu))筑物控制樁復(fù)測報告

齊全;樁位設(shè)置規(guī)范,保護措施滿足要求。

4.0.2施工用原材料、成品、半成品等物料存放管理滿足要求。

4.0.3原材料、成品、半成品質(zhì)量檢驗合格,報告齊全。

4.0.4完成混凝土配合比設(shè)計,其試配強度、耐久性、抗腐蝕性等指標滿足設(shè)計要求。

4.0.5現(xiàn)場混凝土攪拌站條件符合預(yù)拌混凝土標準規(guī)定;預(yù)拌混凝土技術(shù)檢驗合格,報告齊全。

4.0.6鋼筋加工場地、預(yù)制件場地、組裝場地的工作條件和水、電、氣等配置滿足要求。

4.0.7建筑基坑及周邊環(huán)境變形監(jiān)測滿足要求。

5質(zhì)量監(jiān)督檢測

5.0.1開展現(xiàn)場質(zhì)量監(jiān)督檢查時,應(yīng)對有關(guān)檢測試驗報告進行查驗,必要時可進行驗證性抽樣檢測。對

監(jiān)督檢查中發(fā)現(xiàn)有突出質(zhì)量問題、隱患的部位或?qū)こ贪踩杏绊懙闹匾课?、隱蔽工程,以及對檢測成

果存疑或與現(xiàn)場實際情況不符的,由具備資質(zhì)的第三方檢驗檢測機構(gòu)獨立進行檢驗檢測,并出具結(jié)論性意

見。

1總則

1.0.1光伏支架基礎(chǔ)施工(無基礎(chǔ)工程為地基處理施工)達到工程監(jiān)督檢查計劃要求的完成量,且完成

相應(yīng)的質(zhì)量檢測與驗收后,方可申請開展光伏支架安裝前監(jiān)督檢查。

1.0.2光伏支架安裝前質(zhì)量監(jiān)督檢查應(yīng)在主要基礎(chǔ)工程隱蔽前完成。

2監(jiān)督檢查前應(yīng)具備的條件

2.0.1已完地基處理經(jīng)檢測后滿足設(shè)計要求,檢測報告齊全。

2.0.2已完工程施工質(zhì)量驗收己完成,不符合項已整改閉環(huán)。

3責任主體質(zhì)量行為的監(jiān)督檢查

3.1建設(shè)單位

3.1.1本階段工程采用的專業(yè)標準清單已審批。

3.1.2組織完成設(shè)計交底和施工圖會檢。

3.1.3組織進行本階段工程建設(shè)有關(guān)質(zhì)量強制性標準(強制性條文)實施情況的檢查。

3.2勘察單位

3.2.1本階段工程建設(shè)有關(guān)質(zhì)量強制性標準(強制性條文)已執(zhí)行。

3.2.2按規(guī)定參加樁基及地基處理工程施工的質(zhì)量驗收。

3.2.3進行了本階段工程實體質(zhì)量與勘察的符合性確認。

3.2.4勘察技術(shù)交底已完成,交底記錄齊全。

3.3設(shè)計單位

3.3.1設(shè)計圖紙交付進度能保證連續(xù)施工。

3.3.2按規(guī)定進行設(shè)計技術(shù)交底并參加施工圖會檢。

齊全;按規(guī)定完成施工項目質(zhì)量驗收并匯總。

3.4.8質(zhì)量問題及處理臺賬完整,記錄齊全。

3.4.9本階段應(yīng)執(zhí)行的工程建設(shè)有關(guān)質(zhì)量強制性標準(強制性條文)實施情況已檢查。

4.1.5沉降觀測點設(shè)置滿足設(shè)計要求及符合標準規(guī)定,觀測記錄、曲線和成果報告完整,符合標準

規(guī)定。

4.2換填墊層地基

4.2.1換填技術(shù)方案、施工方案齊全,已審批。

4.2.2地基驗槽滿足設(shè)計要求,驗收記錄齊全。

4.2.3砂石、粉質(zhì)黏土、灰土、礦渣、粉煤灰、土工合成材料等換填墊層材料性能滿足設(shè)計要求,質(zhì)

量證明文件齊全。

4.2.4換填土料按標準規(guī)定進行擊實試驗、土顆粒分析試驗及設(shè)計有特殊要求的試驗合格。

4.5.2根據(jù)不同土質(zhì)采取的強夯夯錘質(zhì)量、夯錘底面形式、錘底面積、錘底靜接地壓力值、排氣孔等

施工工藝參數(shù)與設(shè)計(施工)方案一致。

4.5.3強夯過程和強夯置換夯符合標準規(guī)定,并采取了必要的隔震或減震措施。

4.5.4質(zhì)量控制參數(shù)符合技術(shù)方案,施工記錄齊全。

4.5.5承載力檢測結(jié)果滿足設(shè)計要求。

4.5.6施工質(zhì)量的檢驗項目、方法、數(shù)量符合標準規(guī)定,質(zhì)量驗收記錄齊全完整。

4.6復(fù)合地基

4.6.2復(fù)合地基技術(shù)方案、施工方案齊全,已審批。

4.6.3散體材料復(fù)合地基增強體密實,檢測報告齊全。

4.6.4有粘結(jié)強度要求的復(fù)合地基增強體的強度及樁身完整性檢測報告齊全。

4.6.5復(fù)合地基增強體單樁的樁位偏差符合標準規(guī)定。

4.6.6質(zhì)量控制參數(shù)符合技術(shù)方案,施工記錄齊全。

4.6.7承載力檢驗結(jié)果滿足設(shè)計要求。

4.6.8施工質(zhì)量的檢驗項目、方法、數(shù)量符合標準規(guī)定,質(zhì)量驗收記錄齊全。

4.6.9振沖碎石樁和沉管碎石樁符合以下要求:

(1)原材料質(zhì)量證明文件齊全;

(2)施工工藝與設(shè)計(施工)方案一致;

(3)質(zhì)量控制參數(shù)符合技術(shù)方案,施工記錄齊全;

(4)承載力檢測結(jié)果滿足設(shè)計要求;

(5)施工質(zhì)量的檢驗項目、方法、數(shù)量符合標準規(guī)定,質(zhì)量驗收記錄齊全。

4.6.10水泥土攪拌樁符合以下要求:

(1)原材料質(zhì)量證明文件齊全;

(2)施工工藝與設(shè)計(施工)方案一致;

(3)對變形有嚴格要求的工程,采用鉆取芯樣做水泥土抗壓強度檢驗,檢驗數(shù)量、檢測結(jié)果符合

標準規(guī)定;

(4)質(zhì)量控制參數(shù)符合技術(shù)方案,施工記錄齊全;

(5)承載力檢測結(jié)果滿足設(shè)計要求;

(6)施工質(zhì)量的檢驗項目、方法、數(shù)量符合標準規(guī)定,質(zhì)量驗收記錄齊全。

4.6.11旋噴樁復(fù)合地基符合以下要求:

(1)原材料質(zhì)量證明文件齊全;

(2)施工工藝與設(shè)計(施工)方案一致;

(3)質(zhì)量控制參數(shù)符合技術(shù)方案,施工記錄齊全;

(4)承載力檢測結(jié)果滿足設(shè)計要求;

(5)施工質(zhì)量的檢驗項目、方法、數(shù)量符合標準規(guī)定,質(zhì)量驗收記錄齊全。

4.6.12灰土擠密樁和土擠密樁復(fù)合地基符合以下要求:

(1)消石灰性能指標及灰土強度等級滿足設(shè)計要求;

(2)施工工藝與設(shè)計(施工)方案一致;

(3)樁長范圍內(nèi)灰土或土填料的平均壓實系數(shù)、處理深度內(nèi)樁間土的平均擠密系數(shù)、抽檢數(shù)量符

合標準規(guī)定;

(4)對消除濕陷性的工程,進行了現(xiàn)場浸水靜載荷試驗,試驗結(jié)果符合標準規(guī)定;

(5)質(zhì)量控制參數(shù)符合技術(shù)方案,施工記錄齊全;

(6)承載力檢測結(jié)果滿足設(shè)計要求;

(7)施工質(zhì)量的檢驗項目、方法、數(shù)量符合標準規(guī)定,質(zhì)量驗收記錄齊全。

4.6.13夯實水泥土樁復(fù)合地基符合以下要求:

(1)原材料質(zhì)量證明文件齊全;

(2)施工工藝與設(shè)計(施工)方案一致;

(3)夯填樁體的干密度、抽檢數(shù)量符合標準規(guī)定;

(4)質(zhì)量控制參數(shù)符合技術(shù)方案,施工記錄齊全;

(5)承載力檢測結(jié)果滿足設(shè)計要求;

(6)施工質(zhì)量的檢驗項目、方法、數(shù)量符合標準規(guī)定,質(zhì)量驗收記錄齊全。

4.6.14水泥粉煤灰碎石樁復(fù)合地基符合以下要求:

(1)原材料質(zhì)量證明文件齊全;

(2)施工工藝與設(shè)計(施工)方案一致;

(3)混合料坍落度、樁數(shù)、樁位偏差、褥墊層厚度、夯填度和樁體試塊抗壓強度等滿足設(shè)計

要求;

(4)質(zhì)量控制參數(shù)符合技術(shù)方案,施工記錄齊全;

(5)樁身完整性檢測數(shù)量符合標準規(guī)定;

(6)承載力檢測結(jié)果滿足設(shè)計要求;

(7)施工質(zhì)量的檢驗項目、方法、數(shù)量符合標準規(guī)定,質(zhì)量驗收記錄齊全。

1.0.2光伏發(fā)電單元的安裝、調(diào)試達到工程監(jiān)督檢查計劃要求的完成量,且完成相應(yīng)的質(zhì)量檢測與驗

收后,方可申請開展光伏發(fā)電單元啟動前監(jiān)督檢查。

2.0.1分批光伏發(fā)電單元范圍內(nèi)的建筑、安裝工程已按設(shè)計施工、調(diào)試完成,并驗收簽證。

2.0.2啟動驗收組織機構(gòu)已成立,已完工程施工質(zhì)量驗收己完成,不符合項已整改閉環(huán)。

2.0.3計算機監(jiān)控系統(tǒng)已經(jīng)完成調(diào)試。

2.0.4生產(chǎn)運行準備工作已經(jīng)就緒。

3責任主體質(zhì)量行為的監(jiān)督檢查

3.1.1本階段工程采用的專業(yè)標準清單已審批。

2.0.1已完地基處理經(jīng)檢測后滿足設(shè)計要求,檢測報告齊全。

2.0.2已完工程施工質(zhì)量驗收己完成,不符合項已整改閉環(huán)。

3責任主體質(zhì)量行為的監(jiān)督檢查

3.1建設(shè)單位

3.1.1本階段工程采用的專業(yè)標準清單已審批。

3.1.2質(zhì)量管理組織機構(gòu)已建立,質(zhì)量管理人員已到位。

3.1.3地基處理方案已審批,試驗性施工或試樁已完成,提供了檢測試驗報告。

3.1.4組織完成設(shè)計交底及施工圖會檢。

3.1.5組織進行本階段工程建設(shè)有關(guān)質(zhì)量強制性標準(強制性條文)實施情況的檢查。
