Purpose目的This procedure is to assist the owner and work groups of an area to assure that areas conditions are safe to proceed with planned work and that all safety considerations have

5、been adequately addressed.本程序旨在幫助區(qū)域業(yè)主及工作組確認(rèn)該區(qū)域的工作環(huán)境是否安全，可以按計(jì)劃執(zhí)行工作，并充分考慮到涉及的所有安全問題。2. General通則2.1 A work permit will be used for all work done in an area, unless the owner and his workforce do the work. If the owner does the work, a work permit is optional.除非業(yè)主或其工作組從事該項(xiàng)工作時(shí)可任意選擇，任何人執(zhí)行區(qū)域工作都必須使用工作許可。2.

6、2 Some routine and frequent jobs may be excluded from this requirement if a complete written procedure exists.對一些日常及常規(guī)性工作，若有完整的書面程序存在，則可不包括在本程序要求之內(nèi)。2.3 Work permit form consists of two sections. The upper section is designed to show work description and the lower section show the safety preparations

Procedure程序3.1 Responsibility職責(zé)The work group supervisor in charge of the work will initiate work permit. The permit will be approved and signed by responsible supervision in the area where the work will be performed.工作許可由負(fù)責(zé)該項(xiàng)工作的工作組

8、主管填寫，并應(yīng)得到該工作執(zhí)行區(qū)域的主管負(fù)責(zé)人的簽署及核準(zhǔn)。It is a joint effort and responsibility between work group supervisor and owner supervisor to fill out work permit properly. First, work group supervisor fills in the work description section and checks safety preparation items of the permit before taking it to the owner

The owner supervisor needs to review and complete the permit, including specific permit requirement and other safety procedures.正確填寫工作許可是工作組主管及業(yè)主主管共同的責(zé)任。首先，在提交業(yè)主主管之前，工作組主管應(yīng)填好工作描述部分并檢查安全準(zhǔn)備條款。然后業(yè)主將對此工作許可予以審查并完成其余事項(xiàng)，包括一些特別許可要求和其它安全程序。The owner must indicate required preparation of equipment for work

The owner must identify known or suspected contaminates and indicate classification and protection need.業(yè)主必須說明執(zhí)行該工作所需的設(shè)備準(zhǔn)備，辨識可知或可疑的危害源并指明類別及必要的防護(hù)。Supervision of work group must note any additional requirements necessary to perform the work safely and use the permit as a checklist.工作

Specific description of the work should be written on the permit. The following should be addressed in filling out the section of the permit.對工作許可中的工作描述部分進(jìn)行扼要說

12、明，明確描述工作內(nèi)容。在填寫該部分內(nèi)容時(shí)應(yīng)注意以下事項(xiàng)：a. The Work Permit will define the location of the work so that the owner and work group can understand the work area well enough to clearly identify any hazards.工作許可應(yīng)說明工作地點(diǎn)，以便業(yè)主及工作組充分了解工作區(qū)域，從而辨識所有危害。b. The Work Permit will provide enough details of the work so that the o

13、wner can determine what hazards or impact on operations may occur while the work is being performed.工作許可應(yīng)提供工作的充分細(xì)節(jié)，以便業(yè)主確定在工作執(zhí)行中對操作產(chǎn)生的危害或影響。c. Specify any hazardous conditions or hazardous material.確定所有危害情況和危害性物質(zhì)。d. The Work Permit can be filled out ahead of time but can only be signed on the shift t

14、he work is to be performed. It should be signed as close as possible to the start of the job after owner completed the preparation and lockout. This will help the Area owner to co-ordinate activities and prevent exposing workers to hazardous situations.可提前填寫工作許可，但必須在工作執(zhí)行的班次方可簽署。工作許可應(yīng)在業(yè)主完成準(zhǔn)備及上鎖后盡可能接近

15、工作開始時(shí)簽署，這將有助于區(qū)域業(yè)主進(jìn)行協(xié)調(diào)工作以防止員工暴露在危險(xiǎn)情形下。3.3 Safety Preparation Section安全準(zhǔn)備部分The following considerations to be given to filling out the Safety Preparations section. All sections pertinent conditions known or suspected on each job requiring a permit shall be completed.填寫安全準(zhǔn)備部分應(yīng)考慮以下事項(xiàng)。所有與需要許可的工作的可知或可疑的相關(guān)

The work permit is valid for 3 days with

18、the following conditions adhered to:下列情形下，工作許可有效期為三天：- Work permit reviewed by all on-coming shifts.經(jīng)每一班次審查。- All conditions of the original authorization are the same with no exceptions.最初授權(quán)的工作許可條件沒有任何變動。- Re-authorized each 24 hrs by the owner supervisor and work group supervisor.業(yè)主主管及工作組主管每24小時(shí)要重

19、新授權(quán)一次。If any condition has changed, a new permit must be issued to continue the job.若條件有變動，則應(yīng)簽發(fā)新的工作許可。b. Work group shall post one copy of authorized permit at the job site when job proceeds and be removed once the job is completed or stopped.工作組將授權(quán)許可張?zhí)诠ぷ鳜F(xiàn)場，并在工作結(jié)束后取走。c.
