5、c非標(biāo)準(zhǔn)規(guī)格標(biāo)志的加施位置，例如本體，銘牌等。 4.生產(chǎn)廠：生產(chǎn)廠家名稱，應(yīng)按照ccc證書(shū)中生產(chǎn)企業(yè)名稱填寫(xiě)。5.工廠代碼：由認(rèn)證機(jī)構(gòu)發(fā)放，一般在ccc證書(shū)生產(chǎn)廠后標(biāo)注。若未標(biāo)注，應(yīng)咨詢認(rèn)證機(jī)構(gòu)。6.產(chǎn)品名稱及型號(hào)：應(yīng)按照ccc證書(shū)中產(chǎn)品名稱及型號(hào)填寫(xiě)。7.郵寄地址及郵編：接收批準(zhǔn)書(shū)的地址和郵編。8.ccc標(biāo)志單獨(dú)尺寸圖：應(yīng)包括實(shí)際使用時(shí)的顏色及ccc標(biāo)志外環(huán)長(zhǎng)直徑。除特殊式樣外，ccc非標(biāo)準(zhǔn)規(guī)格標(biāo)志應(yīng)與ccc標(biāo)準(zhǔn)規(guī)格標(biāo)志成比例縮放。 9.整體設(shè)計(jì)圖：應(yīng)含有ccc標(biāo)志，通常還包含產(chǎn)品型號(hào)等。標(biāo)志加施位置可以是銘牌、產(chǎn)品本體、包裝。10.設(shè)計(jì)圖對(duì)應(yīng)的ccc證書(shū)編號(hào)：整體設(shè)計(jì)圖

6、所適用的中國(guó)強(qiáng)制性產(chǎn)品認(rèn)證證書(shū)編號(hào)。 提示：1.如代理機(jī)構(gòu)進(jìn)行申購(gòu)（非證書(shū)申請(qǐng)人或生產(chǎn)廠），還需提供ccc證書(shū)申請(qǐng)人或生產(chǎn)廠加蓋公章的委托書(shū)原件，須有授權(quán)人簽字。2.以下產(chǎn)品需要在ccc標(biāo)志單獨(dú)尺寸圖中標(biāo)注工廠代碼：機(jī)動(dòng)車輛及安全附件、電線電纜、機(jī)動(dòng)車輛輪胎、安全玻璃、裝飾裝修材料、玩具、消防產(chǎn)品、安全技術(shù)防范產(chǎn)品。3.若整體設(shè)計(jì)圖在表中無(wú)法完整體現(xiàn)，應(yīng)單獨(dú)提供設(shè)計(jì)圖。 4.相同申請(qǐng)人、生產(chǎn)廠及工廠代碼、標(biāo)志加施方式、加施位置可填一份申請(qǐng)表。 how to fill 1. applicant: company name, please keep in accord

7、ance with ccc certificate. 2. the way of mark processing: in which ways the irregular-size ccc mark be added (such as: pressed, molded, and screen-printed)3. mark position: please indicate where to place the mark4. factory: factory name should be kept in accordance with ccc certificate.5. factory co

8、de: issued by certification body, please keep in accordance with ccc certificate.6. product name and model: the name and model of product showed on the ccc certificate.7. postal address and postal code: address and postal code of receiving irregular-size ccc mark printing/pressing permission. 8. fig
