1、班主任考評(píng)操作辦法一、指導(dǎo)思想為進(jìn)一步提高班主任工作水平,調(diào)動(dòng)我校班主任工作積極性,促進(jìn)德育工作向縱深方向發(fā)展,現(xiàn)結(jié)合江都市教育局關(guān)于加強(qiáng)小學(xué)班主任工作指導(dǎo)意見(jiàn)和我校班級(jí)管理工作的實(shí)際,制定邵伯小學(xué)班主任考評(píng)操作辦法。二、考核原則1、考核要公平、公正、公開(kāi)。2、考核內(nèi)容應(yīng)是基本的、穩(wěn)定的、機(jī)會(huì)均等的、易操作的。3、考核結(jié)果全部以量化形式表示,量化結(jié)果作為發(fā)放班主任津貼的依據(jù)。三、考核項(xiàng)目(一)、班級(jí)簿冊(cè)的檢查(10分)在平時(shí)抽查或統(tǒng)一檢查時(shí),各類班級(jí)薄冊(cè)完成較差者每次扣2分,計(jì)劃、總結(jié)等工作沒(méi)按時(shí)完成各扣5分。(二)、一日常規(guī)評(píng)比(55分)1、安全工作(15分)(1)不能按要求對(duì)學(xué)生進(jìn)行安全教
2、育,班級(jí)內(nèi)由于班主任教育不到位而出現(xiàn)重大安全事故,扣除當(dāng)月考核的全部金額。(2)班級(jí)內(nèi)出現(xiàn)打架、追打等現(xiàn)象,每人次扣1分,如造成人員傷害的扣5分。(3)學(xué)生在校園內(nèi)騎自行車的每人次扣1分,如造成人員受傷的,視班主任教育不到位,并視情節(jié)輕重扣5分或10分。(4)不遵守學(xué)校的安全要求,如出現(xiàn)爬圍墻、爬鐵門等違規(guī)現(xiàn)象的每人次扣1分。2、廣播操(5分)。(1)無(wú)故不做廣播操的班級(jí)每次扣2分。(2)無(wú)故不做的每人次扣1分。(3)進(jìn)場(chǎng)、退場(chǎng)隊(duì)伍不整齊的每次各扣0.5分。(4)做操不認(rèn)真(大聲講話、動(dòng)作嚴(yán)重不到位)的每人次扣0.5分。(5)班主任不跟班,每次扣1分。3、眼保健操(5分)。(1)無(wú)故不做眼保健
3、操的班級(jí)每次扣2分。(2)無(wú)故不做或講話的每人次扣1分。(3)無(wú)值日班長(zhǎng),每次扣1分。(4)眼保健操多數(shù)人動(dòng)作不到位,扣1分。4、路隊(duì)(5分)。(1)無(wú)故不排隊(duì)的班級(jí)每次扣1分。(2)不按要求排隊(duì)和插隊(duì)的班級(jí)每次扣1分。(3)隊(duì)伍中有說(shuō)話聲、拉扯打鬧的或隊(duì)伍散亂的班級(jí)扣1分。(4)無(wú)路隊(duì)長(zhǎng),每次扣1分。5、衛(wèi)生工作(15分)。室內(nèi)衛(wèi)生:(1)不打掃的班級(jí)扣2分。(2)打掃不干凈或不及時(shí)的班級(jí)扣1分。(3)課桌不齊的扣1分。(4)衛(wèi)生角勞動(dòng)用具不整潔或擺放不整齊(包括圖書(shū)的擺放)的扣1分。垃圾較多而未及時(shí)倒掉扣1分。(5)教室內(nèi)多處有紙屑的扣1分。(6)講臺(tái)不整潔的扣1分。窗臺(tái)有灰塵未擦的扣1分
4、。(7)教室墻壁有腳印或有臟痕的每處扣1分。(8)課桌、板凳面或其他地方有墨跡或刻痕的每處扣1分。(9)墻角有明顯蜘蛛網(wǎng)而未打掃的每一處扣1分。室外衛(wèi)生:(1)室外保潔區(qū)不打掃的班級(jí)扣2分。(2)打掃不干凈或打掃不及時(shí)的扣1分。(3)垃圾不倒進(jìn)垃圾箱的扣1分。(4)保潔區(qū)搭界處不清潔的相關(guān)班級(jí)各扣1分。(5)室外墻壁瓷磚有腳印或有臟痕的每處扣1分。(6)室外墻角有蜘蛛網(wǎng)的每一處扣1分。(7)樓梯未掃的扣2分。(8)隨地拋紙屑等雜物的扣2分(包括在學(xué)校大門外的路上、特別是廁所內(nèi)、小店門口)。安全衛(wèi)生日檢查:打掃不合格班級(jí),每次扣2分。6、紀(jì)律(10分)。(1)群體追打扣5分,個(gè)別追打每人扣1分。
5、(2)罵人、講粗話的各扣1分。(3)攀折樹(shù)木花枝的扣1分。(4)玩自來(lái)水的每人扣1分。(5)無(wú)故出校門的扣1分。(6)學(xué)生證、紅領(lǐng)巾佩戴不齊的每人扣1分。(7)帶零食進(jìn)校園的每人扣1分。(8)紅領(lǐng)巾廣播站廣播時(shí)不收聽(tīng)的班級(jí)扣2分,收聽(tīng)情況不好或有教師上課或做作業(yè)的班級(jí)扣1分。(9)其他凡違反校園紀(jì)律的學(xué)生視具體情況進(jìn)行扣分處理。(三)、晨會(huì)、班隊(duì)會(huì)(5分)1、晨會(huì)、班隊(duì)會(huì)沒(méi)舉行的每次扣1分。2、班隊(duì)會(huì)不按要求、沒(méi)有主題每次扣1分。(四)、班級(jí)布置(10分)1、每月按要求出好一期黑板報(bào),不能如期完成每次扣2分,不做的扣5分,檢查評(píng)比為差的扣1分。2、按學(xué)校要求進(jìn)行班級(jí)個(gè)性化布置,不能如期完成每次
6、扣2分,不做的扣5分,檢查評(píng)比為差的扣1分。(五)、教室管理(5分)門窗未關(guān)鎖好的每次扣2分。如造成失竊的,視具體情況由學(xué)校校長(zhǎng)室研究決定。(六)、參加班主任例會(huì)(5分)無(wú)故不參加的每次扣2分,每遲到一次扣1分。(七)、積極參賽(10分)學(xué)校組織的各項(xiàng)活動(dòng),各班級(jí)均能積極參加,無(wú)故不參加,每次扣2分。(八)、加分因素。(1)積極組織學(xué)生參加學(xué)校開(kāi)展的各種活動(dòng),按成績(jī)給班主任加分(只計(jì)算集體加分)。一等獎(jiǎng)加3分,二等獎(jiǎng)加2分,三等獎(jiǎng)加1分。(2)主動(dòng)承擔(dān)觀摩班會(huì)加5分。(3)德育論文在各級(jí)刊物獲獎(jiǎng)?wù)?,指?dǎo)學(xué)生獲獎(jiǎng)?wù)?,全?guó)級(jí)加10分,市級(jí)加5分。(4)主動(dòng)承擔(dān)額外活動(dòng)任務(wù)加2分。四、考核(一)、考
7、核辦法:1、班級(jí)簿冊(cè)的檢查、積極參賽情況、參加班主任例會(huì)情況由少先隊(duì)大隊(duì)部進(jìn)行檢查考核,教室管理、安全衛(wèi)生日檢查由總務(wù)處進(jìn)行檢查考核,晨會(huì)、班隊(duì)會(huì)的正常開(kāi)展、班級(jí)布置由各年級(jí)組長(zhǎng)進(jìn)行檢查考核,一日常規(guī)評(píng)比以紅領(lǐng)巾監(jiān)督崗檢查積分為主要數(shù)據(jù)參照。2、各年級(jí)組長(zhǎng)對(duì)所在年級(jí)班級(jí)管理各個(gè)方面的檢查記載,仍然作為考核的重要參照。(二)、考核說(shuō)明:1、本考核滿分為100分,不含加分因素。2、班主任月考核的情況,由少先隊(duì)大隊(duì)部和各年級(jí)組長(zhǎng)組織實(shí)施,每月由匯總一次少先隊(duì)大隊(duì)部并報(bào)校長(zhǎng)室。每學(xué)期考核分5個(gè)月進(jìn)行,各班主任每月的基本津貼為50元/月,同時(shí)每月按照實(shí)得分情況在年級(jí)組內(nèi)評(píng)出名次,獲得津貼:第一名50元/
