HistoryofpolioeradicationandtransitioninSouthSudan

TheRepublicofSouthSudanhasbeenwildpolio-freesince2009.However,thecountryexperiencessporadicoutbreaksofcVDPVandisconsideredvulnerabletofurtheroutbreaksorimportations.Thehealthsystemisfragile,withfrequentoutbreaksofmeaslesandothervaccine-preventablediseasespointingtosubnationalimmunizationgaps.Theongoinghumanitariancrisisinthecountryhasfurtherimpactedaccesstohealthcare.

SouthSudandevelopedatransitionplantocovertheperiod2018–2022,whichwassupersededbytheRepublicofSouthSudanAddendumforPolioTransitionin2021.ThefocusoftheaddendumistosustainthecriticalfunctionscurrentlysupportedbytheGlobalPolioEradicationInitiativewithinthenationalhealthsystem,namelyessentialimmunization,diseasesurveillanceandemergencypreparednessandresponse,aswellascommunicationsfordevelopment.

Objectives

TheoverallaimoftheGovernmentofSouthSudanistosuccessfullytransitionpolioprogrammeknowledge,infrastructureandassetstobepartofthenationalhealthsystem.TheRepublicofSouthSudanAddendumforPolioTransition,June2021,includescommitmentsto

governmentownershipofpoliofunctions,thefullengagementofabroadrangeofstakeholdersincludingdonorsandcivilsociety,andbalancingpoliotransitionwithafocusoninterruptingandclosingpoliooutbreaks.

Specifically,thetransitionplanaddendumsetsouttheintentiontotransitionpoliofunctionsundertheBomaHealthInitiative.TheBomaHealthInitiativeisanationwidestrategytoimproveaccesstoessentialhealthservices,launchedin2017.Itaimstodeliveranintegratedpackageofhealthpromotionanddiseasepreventionactivitiestocommunities,combiningacomprehensivecommunitysurveillancesystemalongsideprogrammingrelatedtocommunicablediseasesandmaternalandchildhealth.

Journeytotransition

PolioeradicationpersonnelhavelongcontributedtobroaderhealthsystemsstrengtheninginSouthSudan.TheircontributionsincludeprovidinginitialemergencyresponsetoCOVID-193,supportingCOVID-19vaccination4,andrespondingtooutbreaksofothervaccine-preventablediseases.TheplantotransitionundertheBomaHealthInitiativeisintendedtoenableintegrationwithbroaderSouthSudanhealthcarepriorities.Inparticular,theinitiativehasafocusonprovidinghealthcaretohard-to-reachandunderservedpopulations,astrengthofthepolioeradicationprogramme.

Toensurethatlessonslearnedarenotlost,thecountryhasidentifiedtwelvebestpracticesfromthepolioeradicationprogrammeinSouthSudan,includingmaintainingmobiledatamanagementsystems,utilizingkeycommunityinformationstructurestostrengthencommunity-basedsurveillanceforAcuteFlaccidParalysis,integratedservicedeliveryofpoliovaccinealongsideinterventionssuchasde-wormingmedication,useoftransitvaccinationpoints,robustmonitoringandevaluation,andsustainingstrongadvocacyandpoliticalsupport.Thesebestpracticesmayhaveapplicabilitytootherhealthinitiatives.

Sincethepoliotransitionaddendumwasdevelopedin2021,implementationhasbeenchallengingduetocompetingprioritiesandtheongoinghumanitariancrisis.Asof2024,theWHOpolioeradicationprogrammecontinuestomanagepoliofunctionstoensurethatthesearesustainedatthenecessarysensitivityandquality.

Financialsustainability

PoliofunctionsarecurrentlysupportedbytheGlobalPolioEradicationInitiative.Inthelongerterm,itisvitalthatsustainablefinancingissecuredtoenablefunctionstobeintegratedwithinWHO,andeventuallytransitionedtothenationalhealthsystem.ThisisimportanttokeepSouthSudanpolio-freeandcontributetobroaderhealthsystemsstrengthening.

Monitoringandevaluation

TheglobalPolioTransitionMonitoringandEvaluationframeworkconsistsoftwosetsofindicatorstomeasurehealthsystemperformanceinrelationtothepolioessentialfunctions,andthepoliotransitionprocess.Theframeworkaimstoguidedecision-making,facilitateprogress,andenablethemonitoringofthequalityofperformanceofthepolioessentialfunctions.
