14、ouldfocus onforestfire preventi on.Fire綜合服務(wù)中心工程竣工驗收報告一、工程概況1、建設(shè)單位：XX工業(yè)園區(qū)管理委員會2、勘察單位：XX 巖土工程有限公司3、設(shè)計單位：XX建筑設(shè)計院4、監(jiān)理單位：XX有限公司5:施工單位：XX建筑工程有限責(zé)任公司XX 建設(shè)工程有限公司6、建設(shè)規(guī)模：投資為1600 多萬元，建筑面積為6721.08m2。7、結(jié)構(gòu)層數(shù)：主樓為6 層框架結(jié)構(gòu)；附屬樓為2 層框架結(jié)構(gòu)。8、工程特征：XX 工業(yè)園區(qū)綜合服務(wù)中心是財政投資重點項目。主體工程由XX設(shè)計院設(shè)計，室外工程及大門、車庫、鍋爐房由XX建筑設(shè)計院設(shè)計；XX有限公司監(jiān)理，XX建設(shè)工程質(zhì)

15、量安全監(jiān)督站質(zhì)檢， 主體及附屬工程由XX建筑工程有限責(zé)任公司中標(biāo)承建，裝修工程由XX建設(shè)工程有限公司中標(biāo)承建，該項目主樓為 6層框架結(jié)構(gòu)，建筑面積為 4995.21 平米，外墻外保溫，正面及側(cè)面干掛大理石，背面涂料， 斷橋鋁門窗，上人屋面；附屬樓為2 層框架結(jié)構(gòu)，建筑面積1725.87平米，外墻干掛大理石，外墻外保溫，斷橋鋁門窗，不上人屋面；新建大門、車庫、鍋爐房等設(shè)施，水、暖、電設(shè)施齊全。工程于2012年 9月 8日開工， 2014年 12月 5日竣工。二、工程現(xiàn)況本工程已于2014年年底完成了設(shè)計和合同約定的各項內(nèi)容。涉及設(shè)計文件的重大變更，已由設(shè)計單位簽署認(rèn)可文件，并經(jīng)審查機(jī)構(gòu)審查通過。

16、 監(jiān)理單位已對工程質(zhì)量控制資料、工程安全和功能檢驗資料進(jìn)行核查并符合要求。法定報建手續(xù)齊全，專項工程已辦理了各項專項驗收手續(xù)。在2015 年 3 月，建設(shè)單位在施工單位提出的工程竣工報告的基礎(chǔ)上，已組織了勘察單位、設(shè)計單位、施工單位、監(jiān)理單位進(jìn)行了預(yù)驗收，總體評定本工程為合格工程，勘察單位、設(shè)計單位已出具了工程質(zhì)量檢查報告，監(jiān)理單位已出具了工程質(zhì)量評估報告。對工程預(yù)驗收中存在的問題，已督促落實到位。三、驗收依據(jù)經(jīng)審查通過的施工圖設(shè)計文件，現(xiàn)行施工驗收規(guī)范、標(biāo)準(zhǔn)， 法律、法規(guī)、規(guī)章和規(guī)范性文件等。四、竣工驗收擬定時間為2016年 2月 3日。五、驗收的組織形式根據(jù)規(guī)定，我單位組織成立了竣工驗收組

17、，驗收組組長由建設(shè)單位負(fù)責(zé)人xx擔(dān)任，副組長由建設(shè)單位項目負(fù)責(zé)人xx擔(dān)任。成員包括： 主樓勘察單位項目負(fù)責(zé)人 xx、附屬勘察單位項目負(fù)責(zé)人 xx、設(shè)計單 位項目負(fù)責(zé)人xx、監(jiān)理單位項目負(fù)責(zé)人xx、總監(jiān)理工程師xx、專業(yè) 監(jiān)理工程師xx、施工單位負(fù)責(zé)人xx、施工單位項目負(fù)責(zé)人xx、施工 單位項目負(fù)責(zé)人xx、施工單位技術(shù)質(zhì)量部門負(fù)責(zé)人xx。驗收組成員中土建和安裝各專業(yè)齊全，參加竣工驗收的人員資格已核查，符合要求。六、驗收內(nèi)容（一）資料核查：1、 核查施工單位的工程質(zhì)量控制資料、工程安全和功能檢驗資料。2、核查施工單位工程竣工報告、監(jiān)理單位工程質(zhì)量評估報告、勘察單位工程質(zhì)量檢查報告、設(shè)計單位工程質(zhì)量

18、檢查報告。3、核查工程變更及檢查工程變更執(zhí)行情況。（二）實地查驗工程質(zhì)量七、驗收程序1、施工單位在完成工程設(shè)計和合同約定的各項內(nèi)容，自檢合格后向建設(shè)單位提交工程竣工報告。2、建設(shè)單位收到工程竣工報告后，組織勘察、設(shè)計、施工和監(jiān)理單位進(jìn)行工程質(zhì)量竣工預(yù)驗收，勘察、設(shè)計單位出具單位工程質(zhì)量檢查合格文件，監(jiān)理單位出具單位工程質(zhì)量評估報告。3、 工程符合竣工驗收條件要求后，建設(shè)單位組織勘察、設(shè)計、 施工、監(jiān)理等單位和其他有關(guān)方面的專家組成驗收組，制定驗收方案。4、建設(shè)單位應(yīng)當(dāng)在工程竣工驗收7 個工作日前將驗收的時間、地點及驗收組名單書面通知負(fù)責(zé)監(jiān)督該工程的工程質(zhì)量監(jiān)督機(jī)構(gòu)。5、建設(shè)單位組織工程竣工驗收

19、。（1）建設(shè)單位核查驗收組成員的資格。（2）建設(shè)、勘察、設(shè)計、施工、監(jiān)理單位分別匯報工程合同履約和在工程建設(shè)各個環(huán)節(jié)執(zhí)行法律、法規(guī)和工程建設(shè)強(qiáng)制性標(biāo)準(zhǔn)的情safetyis thefirst priority況。( 3)建設(shè)單位主持人宣布驗收方案。( 4)參加驗收人員進(jìn)行工程實物質(zhì)量檢查、查驗有關(guān)工程技術(shù)資料。( 5)參建各方責(zé)任主體分別發(fā)表驗收意見并形成驗收結(jié)論。(6)驗收組組長根據(jù)驗收組統(tǒng)一意見，宣布工程是否合格、通過驗收。(7)工程質(zhì)量監(jiān)督機(jī)構(gòu)監(jiān)督驗收人員宣布該工程的竣工驗收組織形式、 驗收程序是否符合規(guī)定，執(zhí)行質(zhì)量驗收標(biāo)準(zhǔn)情況以及該工程驗收會議是否有效。(8 )對工程勘察、設(shè)計、施工、設(shè)
