,"A3-daychemicalmanagementprogramassessmentforSeacoDongguanwasconductedonDecember14-16,2016.Thepurposeofthisassessmentwastoevaluateeffectivenessofsupplier'schemicalmanagementprogramandtohelpsuppliersidentifyareasforimprovement.Eightelementsofchemicalmanagementwereevaluatedandtheassessmentofeachelementconsistsofdocumentreview,fieldinspection,andpersonnelinterviews.Seaco'sEHSmanagementsystemwasatthebeginningstageandunderdevelopment,andthechemicalmanagementprogramwasbasicandfragmented.Abasicchemicalinventorywasdevelopedandupdatedperiodically.Abasichazardcommunicationprogramwasdevelopedandimplemented.Medicaltestingwasofferedtoaportionofworkersexposedtochemicals&physicalagents.Emergencyresponsedrillsandexerciseswereconductedandsomeemergencyresponseequipment&facilitiesweregenerallymaintainedasscheduled.Examplesofdeviationsfoundduringthisassessmentwere:incompletechemicalinventorywithmissinginformationsuchasjobtaskdescription,maximumallowablestoragelimit,andexposedpopulation,useofApplerestrictedchemicals,nonewchemicalreviewingandapprovingprocess,noriskassessmentonchemicalhazards,noengineeringcontrolprogramandinadequateengineeringcontrolsforhazardouschemicalhandling,missingchemicallabelsandchemicalhazardsnotcommunicatedtoaffectedworkers,nochemicaltrainingprovidedtoworkers,medicalsurveillanceprogramnotimplementedtocoverallaffectedworkersandallrequirements,nochemicalhandlingSOPs,andlackoftrainingonemergencyresponseanddeficienciesintheemergencyresponseplan.

在2016年14-16日，對海宏科技有限公司（以下簡稱海宏公司）進(jìn)行了為期3天的化學(xué)品管理體系評估。其目的是評估其管理體系的有效性和幫助供應(yīng)商找到需改善的地方。評估了化學(xué)品管理的8個要素，每個要素均作了文件查閱、現(xiàn)場調(diào)查和人員訪談。海宏公司的EHS管理體系處于初始建立狀態(tài)，化學(xué)品管理體系非?；竞土闼?。建立了基本的化學(xué)品清單并定期更新，簡歷和貫徹了一個基本的危害告知程序。向部分員工提供了職業(yè)健康體檢。舉行了應(yīng)急演練和維保了部分應(yīng)急響應(yīng)設(shè)備設(shè)施。評估中也發(fā)現(xiàn)了一些不足：化學(xué)品清單不全面，缺失許多重要信息如工作崗位/任務(wù)描述，最大允許儲存量和暴露人群等，使用了蘋果禁止使用的化學(xué)品如正己烷，無新化學(xué)品評估審核程序，未對化學(xué)品的危害進(jìn)行風(fēng)險評估，未建立工程控制程序，危險化學(xué)品操作的工程控制設(shè)施缺失，部分化學(xué)品容器無標(biāo)簽和員工危害告知沒有到位，未給接觸化學(xué)品的員工提供培訓(xùn)，職業(yè)健康體檢只覆蓋部分員工和只針對崗中階段，化學(xué)品操作規(guī)程尚未建立，應(yīng)急培訓(xùn)不足和應(yīng)急預(yù)案諸多方面不完善。",,,,,

風(fēng)險控制","Seacoappliedsomemeasurestocontrolchemicalexposurerisks,eg:PPE.However,therewasnowrittenengineeringcontrolprogramwiththeuseofsubstitution,isolation,containment,andventilationmethods.Thereweresomeworkplaceswithnoanyventilationsystemsfortheoperatorstohandlethosehazardouschemicals.SeacoestablishedthePPEmanagementprocedureandPPEwasprovidedforworkerswhomayexposetochemicals.However,therewasnodocumentaboutPPEneedsassessmentbasedontheriskpresentinworkplaces.Gapswereobservedinrespiratorsandglovesselection,use,care,andmaintenance.Operatorswerenottrainedonhowtowear,careandmaintainPPEproperly,therewasnoestablishedrespiratorcartridgereplaceschedule.About20%ofworkersdidnotweartherequiredPPEduringthefieldinspection.Seacodidnotdevelopamanagementprocedureforfire/explosionprevention.Firefightingpermitforchemicalwarehousewasnotobtained.

危害告知","Seacohasdevelopedahazardcommunicationprogram,andMSDSswereavailablefor90%ofchemicals.However,alltheseMSDSswerenotincompliancewiththeGHSstandard,somehazardousingredientswerenotshownontheMSDSs.ItwasobservedbottledwatercontainerswereusedaschemicalpackagingmaterialsfortheA/Bcleaningagent,manychemicalcontainersdidnothaveformalchemicallabelandproperwarningstatments.Thereareoccupationalhealthriskhazardcommunicationnotificationcardintheworkplace,butthecontentdidnotmatchthechemicalsonsite.SeacodidnotconductthehazardcommunicationtrainingtotherelevantworkersasrequiredintheirHAZCOMprogram.TheinterviewedpersondidnotknowhowtogettheMSDSforthosechemicalshandled,operatorsdidn'tknowthesignsandsymbolsmeaningoftheMSDSandchemicallabels.InterviewedEHSprofessionalswerenotfamilarwithhazardclassificationandriskdetermination.

東莞海宏科技有限公司建立料《危害溝通程序》，約90%的化學(xué)品MSDS可以獲取到，但MSDS都不是符合國家規(guī)范的GHS版本MSDS。工作場所儲存的化學(xué)品存在沒有正規(guī)的化學(xué)品標(biāo)簽、使用礦泉水來盛裝A/B處理劑。許多裝化學(xué)品的容器無化學(xué)品標(biāo)簽和危害聲明。瓶部分工作場所張貼的“職業(yè)危害告知”內(nèi)容不正確。該公司沒有依照《危害溝通程序》中所提及的“每年對相關(guān)員工進(jìn)行危害溝通培訓(xùn)”執(zhí)行，采訪員工發(fā)現(xiàn)，員工尚不清楚應(yīng)該如何獲取到自己操作的化學(xué)品的MSDS，不清楚化學(xué)品標(biāo)簽上面的圖標(biāo)含義的情況。采訪EHS負(fù)責(zé)人，她尚不清楚危險化學(xué)品分類的方法以及如何判別某種化學(xué)品的危險性。

東莞海宏科技有限公司建立了用于監(jiān)控日常運作的各項目標(biāo)指標(biāo)，但這些目標(biāo)指標(biāo)中尚不涵蓋與安全和健康相關(guān)的內(nèi)容，公司沒有配備專職的安全、環(huán)保、健康管理人員。該公司目前尚不具備足夠資源來支持公司的管理層、各級經(jīng)理和主管以及EHS相關(guān)人員獲取到專業(yè)的安全健康管理知識和能力。通過查看培訓(xùn)記錄了解到，該公司進(jìn)行了一些與化學(xué)品管理相關(guān)的培訓(xùn)，但培訓(xùn)材料的內(nèi)容未能涵蓋本次評估的諸多方面，通過采訪員工也印證料培訓(xùn)不到位的情況,該公司仍然需要進(jìn)一步就化學(xué)品清單、風(fēng)險評估、醫(yī)療監(jiān)護(hù)、化學(xué)品操作等方面做深入的培訓(xùn)。",,,

健康監(jiān)護(hù)","Seacoestablishedamedicalsurveillanceprogram,however,theactualimplementationwasnotincompliancewiththewrittenprogram.Seacoonlyofferedthemedicalsurveillancetestingtoaportionofworkerswithexposurestothechemicalandphysicalagentssuchasnoise.Seacodidnotprovidepre-placementandexitmedicaltestsfortheworkersthatshouldbeincludedinmedicalsurveillanceprogram.Seacodidnotdevelopapreparationinstructionbasedonvariousmedicaltestingrequirementsandcommunicatedthepreparationreuirementstoaffectedworkers.Interviewedworkerswerenotnotifiedoftheresultsandsignedonthereportstoacknowledgethereceiptofthereports.Follow-upexaminationswerenotarrangedforworkerswithabnormaltestingresults.Theincidentofleftfootinjury,happenedonFeb.26,2016,wasnotinvestigatedandincidentinvestigationreportwasnotfoundandcorrectiveactionswerenotdevelopedandimplemented.Medicalsurveillanceprogramadministratorwasnotfamiliarwithrelevantregulations,standards,andpractices.
