5、胞、間質(zhì)細(xì)胞和胎兒血管內(nèi)皮細(xì)胞; T E M發(fā)現(xiàn), 1例未接受抗病毒藥物治療的H I V - 1感染孕婦胎盤滋養(yǎng)細(xì)胞內(nèi)有約100nm大小的病毒樣顆粒結(jié)構(gòu)存在,可見病毒進(jìn)入細(xì)胞的吞噬小泡,病毒樣顆粒周圍溶酶體聚集,細(xì)胞有內(nèi)質(zhì)網(wǎng)擴(kuò)張和空泡樣改變,間質(zhì)H of ba u e r細(xì)胞增多。其余陽性胎盤T E M下沒有發(fā)現(xiàn)類似病毒顆粒與超微結(jié)構(gòu)的改變。2. 病例組胎盤中的C D 3、C D 68陽性細(xì)胞數(shù)均高于對照組(P 0.001),200倍視野下兩組的C D 3、C D 68陽性細(xì)胞分別為3.61.1 v s 1.00.6和20.52.7 v s12.23.4;I L - 2 在滋養(yǎng)細(xì)胞層和絨毛間質(zhì)

6、弱表達(dá),兩組間差異無統(tǒng)計學(xué)意義;病例組I L - 4的表達(dá)低于對照組(P 0.05),兩組的平均P U值為2.510.75 v s3.220.71; T N F - a的表達(dá)那么高于對照組( P 0.05),兩組的平均P U值為3.861.08v s 3.010.86。3. C D 4在早孕絨毛和晚孕胎盤中的表達(dá)存在個體差異,早孕組陽性率為70%(7/ 10),晚孕對照組為61.54%(8/ 13),病例組為72.73%(8/ 11),三組的陽性率差異無統(tǒng)計學(xué)意義。 C X C R 4、 C C R 5及D C - S I G N分子在胎盤均有表達(dá),三者均定位于滋養(yǎng)細(xì)胞和絨毛間質(zhì)細(xì)胞,且在早孕

8、論:1. 本研究從組織學(xué)形態(tài)層面證實,在 H I V - 1感染孕婦,病毒可以感染胎盤的滋養(yǎng)細(xì)胞,且胎盤的超微結(jié)構(gòu)有所改變;病毒在胎盤中主要定位于滋養(yǎng)細(xì)胞、胎盤的間質(zhì)細(xì)胞和胎兒血管內(nèi)皮細(xì)胞。但是,在接受抗病毒藥物治療的 H I V - 1感染孕婦胎盤中沒有發(fā)現(xiàn)類似病毒顆粒與超微結(jié)構(gòu)的改變,提示產(chǎn)前干預(yù)在阻斷HIV-1感染垂直傳播可能具有重要意義。2. 胎盤受到 H I V - 1感染時,細(xì)胞免疫可能有所增強(qiáng); I L - 4表達(dá)降低, T N F - a表達(dá)增加,提示 H I V - 1的感染可能促使母胎界面向免疫排斥轉(zhuǎn)變,對妊娠結(jié)局可能造成不利影響。3. 胎盤組織中存在 H I V - 1各

9、種受體分子,具備感染 H I V - 1的分子根底。胎盤中 C D 4分子的表達(dá)存在個體差異; C C R 5、 C X C R 4及 D C - S I G N 分子在晚孕胎盤中的表達(dá)較高,可能是HIV-1感染宮內(nèi)傳播多發(fā)生于孕晚期的原因之一。4. 滋養(yǎng)細(xì)胞系不易被游離的 H I V - 1感染,與被感染的 T 細(xì)胞系共培養(yǎng)可以提高滋養(yǎng)細(xì)胞易感性。病毒在滋養(yǎng)細(xì)胞中存在復(fù)制。滋養(yǎng)細(xì)胞系外表低水平表達(dá)的 C C R 5、C X C R 4和 D C - S I G N可能與病毒進(jìn)入滋養(yǎng)細(xì)胞的機(jī)制無關(guān)。關(guān)鍵詞:H I V - 1;胎盤; L S C M; T E M; C D 3; C D 68;
