8、作 人員一份。北京首都航空小動物運輸申請書反面APPLICATION FOR PETS TRANSPORTION OF CAPITAL AIRLINES一. 小動物是指家庭馴養(yǎng)的小狗、貓。二. 小動物運輸限制和條件1、提前48小時向首都航空售票單位進(jìn)行申請。2、除導(dǎo)盲犬、助聽犬及救助犬等工作犬外均不能作為客艙行李運輸，必須辦理托運。3、每名旅客限制僅能運輸一只小動物。4、 小動物作為托運行李運輸時，寵物和籠子的總重量重量不得超過20Kg。5、導(dǎo)盲犬、助聽犬、救助犬等工作犬運輸不受本條 3及 4款規(guī)定限制。6、對于出現(xiàn)的以下情況，承運人無法辦理航空運輸：托運人所托運的活體動物為國家禁運的動物之列

9、。具有傳染 病征候，或疑似傳染病載體的動物如發(fā)生地區(qū)禽流感的家禽。犬類動物及貓科動物年齡小于六周。懷孕動物或是 在飛機起飛前48小時之內(nèi)剛剛分娩過的動物。需要托運人完成的動物飛行運輸準(zhǔn)備未完成。運輸過程中可能產(chǎn)生 的承運人認(rèn)為的天氣、高熱氣溫通常高于 85 ° F/29 C的往返機場等不適于哈巴狗或扁平鼻的狗或貓運輸?shù)钠渌?原因。因航班機型原因，不適于運輸活體動物。如貨艙無適運艙位不符合始發(fā)、經(jīng)停、目的地城市或國家動物運輸?shù)南嚓P(guān)規(guī)定航空公司工作人員判斷該動物有可能對旅客和/或其行李造成損害，或者航空運輸有可能對該動物造成損害。三、運輸文件要求1、乘坐國際航班，旅客應(yīng)在至少出發(fā)前 7天

10、為寵物備妥以下文件 相關(guān)政府目的地/中轉(zhuǎn)地核發(fā)的有效輸出/輸入文件。 有關(guān)當(dāng)局核發(fā)的有效健康聲明書及狂犬病疫苗注射證明書。 妥善的備齊所有行程中含蓋的入境/過境國要求的入境許可、健康聲明書及疫苗注射證明。 任何行程中含蓋的入境/過境國政府要求的額外特殊文件2、國內(nèi)運輸所須文件動物衛(wèi)生監(jiān)督所出具的動物檢疫合格證明?，其中“運載工具消毒情況一欄必須填寫且單據(jù)上需蓋有動物衛(wèi)生監(jiān)督 所檢疫專用章。3、 導(dǎo)盲犬、助聽犬、救助犬等工作犬運輸所須文件：除須備齊本條1或2款證件外，還應(yīng)攜帶有效的動物訓(xùn)練合格證 明書?、動物工作證?或/和動物身份證?。四、寵物箱要求1、首都航空建議旅客使用專用航空箱運輸動物。2

11、、寵物箱必須由鞏固材料制成且頂部固定。乘坐國內(nèi)航班至少三面通風(fēng)，國際航班四面通風(fēng)，且門必須由金屬制成。硬件包括螺母、螺絲以及門確保安裝完整。箱門牢固須有鎖閉裝置，有固定的把手或凸起邊緣。寵物箱的空間 應(yīng)足夠大，可以保證寵物自由站立或坐下、轉(zhuǎn)身和以正常姿勢躺臥。寵物箱的底部平穩(wěn)，能夠固定在平整的面上而 不滑動。帶輪子的寵物箱，需要進(jìn)行必要的固定。保證在運輸過程中輪子不滑動.寵物箱必須經(jīng)過打包處理，中號箱每面至少捆扎2匝，呈“井字形，大號箱每面至少捆扎 3匝，打包帶分布均勻，保證在運輸過程中正常情況下打 包帶不脫落。3、寵物箱箱內(nèi)須鋪上吸水性襯墊，例如毛巾、毯子或者白色的紙，以防止寵物排泄物外溢污
